Although BP has established several reporting systems such as Tr@ction,Near Miss <strong>Report</strong>s, and Lessons Learned, some hourly workers at Carson, Toledo, and Whiting complained about the difficulty in using Tr@ctionand its poor design. 72 In addition, some survey respondents expressed dissatisfaction with the safety reporting system, primarily at Texas City andToledo. For example, almost one-third of operators at Texas City (32 percent) and more than one-third of operators at Toledo (38 percent) indicatedthat they were not satisfied with the process safety reporting system at their refineries. More than one-third of the maintenance/craft techniciansat Texas City (35 percent) and Toledo (40 percent) 73 expressed similar dissatisfaction, as did 36 percent of HSSE employees at Texas City and26 percent of HSSE employees at Toledo. 74 <strong>The</strong>se concerns were less prevalent at Carson and Cherry Point, where no more than 15 percent ofBP employees in the process safety functional groups and contractors responded negatively.Finding:BP’s process safety management system likely results in under reporting of incidents and near misses atBP’s five U.S. refineries.Performance Evaluation, Corrective Action, and Corporate Oversight C 208
