1. IntroductionOn March 23, 2005, the BP Texas City refinery experienced an explosion and fire thatresulted in 15 fatalities and injured over 170 BP employees and contractors. The incidentinvolved a vapor cloud explosion resulting from a release of flammable material from araffinate splitter tower and the associated blowdown system. 1The U.S. Chemical Safety and Hazard Investigation Board (CSB) and other federal, state,and local agencies immediately began an investigation. The CSB subsequently issued tothe BP Global Executive Board of Directors an urgent recommendation to commission anindependent panel to evaluate the effectiveness of BP Products North America’scorporate oversight of safety management systems at its U.S. refineries. 2 BP acceptedthis recommendation and formed the BP U.S. Refineries Independent Safety ReviewPanel (Independent Panel) on October 24, 2005. 3In January 2006, the Independent Panel retained ABSG Consulting Inc. (ABSConsulting) and JL McCavit Consulting, LLC (collectively, the PSM Review Team) toconduct process safety management (PSM) reviews at BP’s refineries in the UnitedStates. This is the final report of the PSM Review Team submitted to the IndependentPanel.9
