9.24 The transition of people with mental health disabilities from mental health facilities tomore appropriate community settings should reflect existing good practice and must besupported by all agencies involved in the delivery of services to people with disabilities.9.25 The following case studies are examples of how agencies are working to address thehousing needs of people with mental health disabilities, including those leaving mentalhealth facilities as provided for under A Vision for Change.Case Study 9.1: HSE South 81In 2007 the HSE South drew up a 5 year plan for the development of mental healthservices in County Wexford. The plan had 2 main elements, the development ofcommunity mental health services and the closure of St Senan’s Hospital.During the period 2007 – 2010, service provision at St Senan’s Hospital was reducedthrough the relocation of residents to more appropriate accommodation in the communityand the reduction of the number of hospital beds from 171 to 85. Simultaneously,community mental health services were developed and include, inter alia:• New Community Mental Health Centres opened in New Ross and Gorey;• Relocation of 22 elderly long stay residents to nursing home care with relevantsupports and the redeployment of staff to community mental health servicesdevelopment;• Extension of day care services in Wexford town and Enniscorthy;• Closure of one intellectual disability unit in St Senan’s Hospital and the residentsrelocated to a purpose built unit at Oylegate, with the assistance of Wexford MentalHealth Association.The most recent report from the Inspector of Mental Health Services in 2010 recognisedthat considerable work has been completed to close wards in St Senan’s Hospital andprovide appropriate alternative accommodation in the community to develop communitybased services.During 2011/2012, HSE South will complete the process for the total closure of long stayaccommodation at St Senan’s Hospital with the implementation of a significant capitalinvestment programme and community based services development, including the openingof a purpose built 12 bed high support/rehab hostel in Enniscorthy, which is expected to beavailable for occupation by end 2011. Funding has been secured for the development of afurther 3 high support residences in Enniscorthy. A Community Nursing Unit is being81Source: HSE110
developed in the grounds of Wexford General Hospital and is expected to be available foroccupation in late 2012. Admissions to St. Senan’s Hospital ceased on 28 February asrequired by the Mental Health Commission.The HSE South’s plan has ensured that reorganisation of acute services will enable serviceusers to be treated closer to their homes whilst maintaining clients’ links to their familiesand communities minimising the impact on their daily lives whilst reducing the number ofacute beds by up to 30%.Case Study 9.2: Monaghan County Council and St. Davnet’s Hospital 82St Davnet’s Hospital is a mental health services hospital in Monaghan with two long-staywards, one admission unit and home-base treatment, rehabilitation and psychiatry of laterlife teams. When an individual is due to be given a discharge date from St Davnet’s and ahousing need is identified, Monaghan County Council is notified and the local authoritysocial worker will visit the individual to discuss his/her needs. The hospital draws up a careplan, with the patient present, in agreement with other service providers (e.g. localauthority, community-based support providers). A holistic approach is taken where theindividual’s proximity to family members, the local town, education and work areconsidered.From the local authority perspective, the intent is to ensure that a successful tenancyensues, and therefore the individual’s housing need is considered in the context ofsupports being available (this would include, for example, providing a second bedroom fora carer) and placement where a person would not experience harassment by othertenants. More often other tenants can be very helpful, acting to identify problems with aperson who has experienced mental illness and communicating this through communitygroups and residents’ associations – this was considered invaluable by the local authorityand the mental health team as it was felt that early identification could prevent majorproblems for the individual at a later date.Not everyone on the housing list with mental health disabilities will be accessing housingfrom the hospital.
