Appendix 6 IndividualAssessment of Need ProtocolProtocol to govern arrangements for cooperation by the Health Service Executivewith housing authorities in relation to the development and coordination of theservices provided by housing authorities for persons with disabilities, aged under5 years, who have been identified by the Health Service Executive under theAssessment of Need process consistent with Section 8 of the Disability Act 2005,and made known to the housing authorities under the provisions of Section 12 ofthe Disability Act, 2005• Where a person (aged under 5 years of age) has been assessed under Section 8 ofthe Disability Act 2005, which commenced on 1 June 2007, and has been identifiedas likely to require housing support, a Liaison Officer 125 , established under Section 11of the Disability Act 2005, shall refer the person to the Director of Services of theHousing Section of the relevant Housing Authority, for the purpose of facilitating thedetermination of any housing service that the Housing Authority considers the personto be entitled to.• An Assessment Officer, established under Section 8 of the Disability Act 2005, mayrefer a person with a disability to the Director of Services of the Housing Section ofthe relevant Housing Authority, if she or he deems it appropriate that the person,who has been identified as likely to require housing support, is made known to thehousing authority as early as possible following the completion of the assessmentunder Section 8 of the Disability Act 2005. In such instances, the Assessment Officershall notify the appropriate Liaison Officer that a referral to the Housing Authorityhas taken place.• The Director of Services of the Housing Section may designate an appropriate officialfor the purpose of engaging / liaising with an Assessment Officer or a Liaison Officer.• Under Section 12 of the Disability Act 2005, the Health Service Executive shall, withthe consent of the individual or, if appropriate, a specified person 126 , provide allrelevant information in relation to the housing element of the assessment report,produced under Section 9 of the Disability Act 2005, to the relevant HousingAuthority for the purpose of assisting the person with a disability in accessing theappropriate housing services provided by the Housing Authority.125Liaison officers are known as case managers in the HSE126A specified person means• A spouse, parent or relative of the person with a disability;• A guardian or a person acting in loco parentis to the person with a disability;• A legal representative of the person with a disability; or• A personal advocate assigned by the Citizens Information Board [formerly Comhairle] to represent theperson with the disability.153153
