may refer the person <strong>with</strong> a mental health disability to the relevant housing authority<strong>for</strong> such purpose.5. The relevant housing authority may designate, <strong>with</strong> the agreement of the person<strong>with</strong> the disability, an appropriate official <strong>for</strong> the purpose of engaging / liaising <strong>with</strong>the key worker in the HSE. This official, as designated, shall be the liaison officer <strong>for</strong>both the key worker in the HSE and the person <strong>with</strong> a mental health disability andtheir advocate, if applicable.6. For the purpose of assisting the person <strong>with</strong> a mental health disability in accessingthe appropriate housing services provided by the housing authority, the HSE shall,<strong>with</strong> the consent of the person <strong>with</strong> the mental health disability and, if applicable,their advocate, provide to the housing authority all relevant in<strong>for</strong>mation, includingmedical evidence outlining the impact that the person’s disability has on their housingneed. The HSE will participate in the housing assessment to the extent that this canfacilitate finding the most appropriate housing service <strong>for</strong> the individual.7. The housing authority shall engage <strong>with</strong> the person who has been identified by theHSE as having a mental health disability, or, if appropriate, <strong>with</strong> the HSE casemanager responsible <strong>for</strong> the arrangement of the service provision, or <strong>with</strong> anadvocate of the person’s choice, if appropriate, <strong>for</strong> the purpose of facilitating orcoordinating the provision of any housing services that the housing authorityconsiders the individual to be eligible <strong>for</strong>, and in need of.8. The relevant housing authority shall provide all appropriate in<strong>for</strong>mation regarding thefull range of relevant housing services available to the person <strong>with</strong> a mental healthdisability who has been considered to have a need <strong>for</strong> housing support, and ifapplicable, to their advocate, or to the HSE.9. Where a person presenting to a housing authority, <strong>for</strong> the purpose of availing of anyrelevant housing services, in<strong>for</strong>ms the housing authority that they have a mentalhealth disability, and(a)there is no medical evidence available to support the person’s housingapplication, or(b)it is determined that the person is not in receipt of any relevant HSE healthand personal social services;161
