CO-CURRICULAR LIFEThe documentation must be no more than three years old, if thestudent was tested before age 21 and within a five year period inthe case of adults tested after age 21. Documentation mustcontain the date of assessment and the diagnosis. It must list theassessment tools utilized describe the functional limitations ofthe disability and support the need for specific accommodations.The documentation must include relevant recommendationsregarding the curriculum, as well as testing considerations. Noaccommodation will be assured until the appropriatedocumentation is on file. <strong>Lake</strong> <strong>Erie</strong> <strong>College</strong> reserves the right torequire that a student provide more complete and/or updateddocumentation at any time. All records are confidential.It is the responsibility of students with disabilities to seekavailable assistance at the <strong>College</strong> and to make their needsknown to the Director of the Student Success Center. Thestudents must meet with the Director upon admission to the<strong>College</strong> or when a disability becomes known to discuss anyspecial needs. All assistance/accommodations are coordinatedthrough the Director of the Student Success Center.Accommodations are reviewed each semester by the Director,the student and a consultant in the area in which the disabilityoccurs, if necessary. While <strong>Lake</strong> <strong>Erie</strong> <strong>College</strong> will providereasonable accommodations, academic success is the student’sresponsibility. Instructors will be expected to assist with theprovision of accommodations when reasonable and necessary.Instructors are not expected to compromise essential elementsof the course or evaluation standards. Some of theaccommodations which may be available to students withdocumented disabilities include textbooks on CD, peer andprofessional tutoring, additional time for testing, testing in adistraction-free environment, small study groups, peer andprofessional editing, time-management and organizationassistance, and personal coaching for success. For furtherinformation contact the Director of the Student Success Centerat 440.375.7426.Health and Wellness ResourcesThe <strong>Lake</strong> <strong>Erie</strong> <strong>College</strong> Student Success Center offers a wide rangeof opportunities to the campus community in the area of healthand wellness. Health and wellness services available on campusinclude a licensed family physician available for appointments oncampus each week, social and academic counseling, academicsupport and monitoring, as well as referrals for dental assistanceand psychiatric services as needed.In addition, assistance to students with disabilities and referralsto community mental health agencies is available upon request.The staff provides screenings for ADHD/ADD, depression, anxietyand post-traumatic stress disorder as well as referrals to outsideagencies for drug and alcohol rehabilitation as needed. Inaddition, referrals for individual counseling and psychologicalservices in many areas are available upon request, as well assupport for student athletes in many areas of academic andsocial life.Residential students are strongly encouraged to make theStudent Success Center staff aware of all on-going health andwellness issues as well as any medications prescribed and takenregularly. The staff is happy to collaborate with your currenthealthcare and/or counseling provider to assist you in locating alocal provider near the <strong>Lake</strong> <strong>Erie</strong> <strong>College</strong> campus. Confidentialitywill be maintained at all times and students, while not required,will have the opportunity to sign release forms for collaborationwith parents, personal healthcare and counseling providers toassure the highest level of the continuity of care while at <strong>Lake</strong><strong>Erie</strong> <strong>College</strong>.All residential students, student athletes and equestrian studentsare required to provide proof that they are covered by a healthinsurance plan.The TriPoint Medical Center and University Hospitals ofCleveland are available to treat any student upon request.Students under the age of 18 must have an authorization on filein the Student Success Center should the need for emergencytreatment arise.Career Services and InternshipsThe Office of Career Services and Internships empowers studentsto build bridges to their future careers by providing resources,programs and services that enable students to translate theiracademic and co-curricular experiences to the world of work in adiverse global environment. Services include personalized careercoaching as well as workshops touching on topics such as:resume writing, internships, interview and job search techniques,dressing for success, etiquette and preparing for graduate orprofessional school.The Office of Career Services and Internships also coordinatesthe process for internship registration. Although not all majorsare required to register for internships, all students areencouraged to register for internships to integrate academicswith their career goals.
