All full-time, residential, and undergraduate students are required to select one <strong>of</strong> these mealplan options (unless residing in the Park Village apartments). The particular meal plan a studentsigns up for is encoded on his or her ID card. All students will be required to present their ID cardto use their meal plan. Only students on one <strong>of</strong> the meal plans or paying cash for a meal at thedoor are allowed in the Dining Hall. Each meal, whether breakfast, lunch, or dinner, is deducted fromthe total number <strong>of</strong> meals available. Meals are for the sole use <strong>of</strong> the individual named on the mealplan and are to be consumed in the Dining Hall. No food or beverage is allowed to leave the DiningHall without prior approval. Unused meals are not carried over and are not refundable. Meal plansare for the Dining Hall, the Snack Bar, or the Health Bar during regular hours.Sack Lunches: If a student on any <strong>of</strong> the meal plans has a class or work schedule that extendsthrough any <strong>of</strong> the serving hours, a sack meal or carry-out tray may be requested for that meal.(Please bring a copy <strong>of</strong> your class/work schedule for verification purposes.) Sack meals mustbe requested 24 hours in advance and may be picked up in the kitchen after 7:15 a.m.,Monday-Friday, and after 10:00 a.m. on weekends.Catering: AmeriServe provides catering for <strong>University</strong> functions sponsored by the administrationand student groups. Arrangements for catering lunches, dinners, picnics, party snacks, andbeverages are made directly with the Executive Chef after completion and approval <strong>of</strong> theappropriate Events Registration Form on the Campus Portal under “Services.”Health Bar: The Health Bar is located in the Chlapaty Recreation and Wellness Center and <strong>of</strong>fersa variety <strong>of</strong> salads, sandwiches, smoothies, and healthy snacks and drinks.Snack Bar: The Snack Bar is located in the Underground in the lower level <strong>of</strong> Peters Commonsand is open Monday through Friday, 7:30 a.m. – 8:00 p.m. <strong>Student</strong>s are encouraged to addmoney to their student ID card for use in the Snack Bar by stopping at the <strong>Student</strong> AccountsOffice located on the third floor <strong>of</strong> Myers Teaching & Administrative Center.Special Diets: If you have any special dietary needs, please personally meet with the ExecutiveChef to tailor a menu to fit your needs.Health Insurance 563/589-3244The <strong>University</strong> <strong>of</strong> <strong>Dubuque</strong> does not require pro<strong>of</strong> <strong>of</strong> health insurance for enrolled students withthe exception <strong>of</strong> international students, student-athletes, and seminary students. However, it ishighly recommended that all enrolled students have health insurance coverage either throughtheir family’s medical plan or some privately purchased health insurance plan.All international students must purchase <strong>University</strong>-approved health insurance with repatriationand medical evacuation coverage.All students planning to participate in intercollegiate athletics at the <strong>University</strong> must provide pro<strong>of</strong><strong>of</strong> health insurance (a current copy <strong>of</strong> insurer’s benefit card), as well as the results <strong>of</strong> a physicaltaken after June 1 <strong>of</strong> the academic year in which they will participate in a sport. A student’s abilityto play an intercollegiate sport at the <strong>University</strong> rests on these documents being received by the<strong>University</strong> Medical Coordinator.48
