First Offense: The student will be required to meet with the instructor to discuss the allegedAcademic Dishonesty <strong>of</strong>fense. The instructor will determine a finding <strong>of</strong> guilt or innocence. If theallegation is substantiated, a grade <strong>of</strong> zero will be recorded for the quiz, test, examination, or workin courses with a letter grade, and “No Credit” will be recorded for the quiz, test, examination, orwork in Credit/No Credit grade courses. If the student fails to meet with the instructor, the matterwill be resolved with the evidence presented and the student will lose the right to appeal the caseany further.After meeting with the instructor, the accused student may request an appeal <strong>of</strong> the unfavorablefinding. An alleged first <strong>of</strong>fense <strong>of</strong> academic dishonesty is appealable to the Associate Dean forAcademic Affairs (college student) or Graduate Program Director (graduate student). A studentchoosing to appeal will have 15 calendar days from the date on which the finding is made toschedule an appointment to meet with the appropriate <strong>of</strong>fice. The student should be prepared toprovide evidence during the meeting to explain why the finding is false. Before making a decisionon the appeal, the Associate Dean or Graduate Program Director will discuss the student’s appealwith the faculty member. The decision <strong>of</strong> the Associate Dean for Academic Affairs (college student)or Graduate Program Director (graduate student) is final.Second Offense: After receiving notification <strong>of</strong> an allegation <strong>of</strong> a second <strong>of</strong>fense <strong>of</strong> academicdishonesty, the student will be required to meet with an Associate Dean for Academic Affairs (collegestudent) or Graduate Program Director (graduate student). After examining the evidence provided bythe instructor and discussing the matter with the student, the Associate Dean for Academic Affairs(college student) or Graduate Program Director (graduate student) will determine a finding <strong>of</strong> inviolation or not in violation. If the student fails to appear at the meeting, the matter will be resolvedwith the evidence presented, and the student will lose the right to appeal the case.If the allegation is founded, a grade <strong>of</strong> “F” will be recorded for the course in which the <strong>of</strong>fenseoccurred for courses with a letter grade, and “No Credit” will be recorded for Credit/No Creditgrade courses. In addition to the above sanction, additional sanctions may be prescribed, such asineligibility to represent the <strong>University</strong> in public performances and/or participate in recognized<strong>University</strong> activities (intercollegiate sports events, choir, student government, or other co-curricularprogram) for a specified period <strong>of</strong> time while enrolled at the <strong>University</strong> <strong>of</strong> <strong>Dubuque</strong>.After meeting with an Associate Dean for Academic Affairs (college student) or Graduate ProgramDirector (graduate student), the accused student may request an appeal to an unfavorablefinding. A second alleged <strong>of</strong>fense <strong>of</strong> Academic Dishonesty is subject to appeal by the accusedstudent to the Academic Standing and Admission Committee (college student) or Vice Presidentfor Academic Affairs (graduate student). An appeal must be submitted in writing to the chairperson<strong>of</strong> the Academic Standing and Admission Committee (college student) or to the Vice President forAcademic Affairs (graduate student) clearly explaining the reasons for and basis <strong>of</strong> the appealalong with evidence the appellant expects to submit at the hearing <strong>of</strong> the appeal. <strong>Student</strong>s have15 calendar days from the date on which the finding is made to submit an appeal to theappropriate <strong>of</strong>fice. Once an appeal has been submitted, the faculty member will be invited torespond to the appeal documents, and then a hearing will be scheduled with the AcademicStanding and Admission Committee (college student) or Vice President for Academic Affairs(graduate student). The decision <strong>of</strong> the Academic Standing and Admission Committee (collegestudent) or Vice President for Academic Affairs (graduate student) is final.81
