Medical Health Services 563/589-3244Finley Occupational Health, which is a service <strong>of</strong> The Finley Hospital, Iowa Health System,provides UD students with access to the services that Finley Hospital has to <strong>of</strong>fer. UD HealthService is available to all students who are currently enrolled in classes. Undergraduate,graduate, and Seminary students are able to seek medical attention with UD Health Service nomatter what insurance plan they are enrolled in. The services also include being seen at FinleyConvenient Care after hours and on weekends. Visits to Finley Occupational Health, which includean assessment by a nurse practitioner or physician, are provided free by UD. Tests, such as labs,x-rays, and visits to the Emergency Room are the responsibility <strong>of</strong> the student. All charges will befiled with the student’s insurance plan.The UD Medical Coordinator is able to schedule appointments not only with Finley OccupationalHealth, but also with other medical facilities. The Medical Coordinator can assist the student withother medical appointments (if requested and within the constraints <strong>of</strong> the student’s personalmedical insurance plan) such as dental, women’s health concerns, specialty clinics, etc. TheMedical Coordinator is also able to help answer general insurance questions.It is imperative that students always have their insurance card and student ID with them formedical appointments and emergency room visits.Transportation to medical facilities: <strong>Student</strong>s are encouraged to organize rides with friends if theycannot transport themselves. Taxi services are available in <strong>Dubuque</strong> if students cannot find othermodes <strong>of</strong> transportation. The <strong>University</strong> does not provide transportation to or from medical facilities.Making a Medical Appointment:1. To schedule a medical appointment for an illness or injury, call the UD Medical Coordinator at563/589-3244.2. The Medical Coordinator will complete a <strong>Student</strong> and <strong>Student</strong>-Athlete Medical Referral whichyou will take to your appointment.3. The Medical Referral form will indicate when the student may return to class or if the student isbeing referred to another physician for follow-up. The student must return the completedreferral form to the UD Medical Coordinator directly after the appointment (if after hours, thenthe next day).4. If you are unable to reach the Medical Coordinator after hours, report to Finley ConvenientCare. If it is an emergency, contact UD Security, ext. 3333, or call 911. Finley Convenient Carewill determine if you should be seen in the Emergency Room (charges for ER services will bebilled to you or your insurance plan).
