37. Patient with <strong>Groin</strong> <strong>Pain</strong> After<br />

a Plug and Patch Hernia Repair<br />

Postoperative groin pain following hernia repair is common. For<br />

most, the pain is acute and resolves with time; however, some patients<br />

develop chronic inguinal pain, termed inguinodynia. Inguinodynia may<br />

occur after any type <strong>of</strong> hernia repair. One <strong>of</strong> the most common inguinal<br />

hernia repair techniques is the plug and patch mesh herniorrhaphy.<br />

Though extremely popular, this procedure has its own set <strong>of</strong> potential<br />

complications that may lead to inguinodynia.<br />

Chief Complaint<br />

Extreme pain when pressing bump in groin<br />

History<br />

A 60-year-old male presents with a history <strong>of</strong> a prior open right inguinal<br />

hernia repair with the plug and patch technique 4 years earlier. His<br />

past medical and surgical history is otherwise not significant except for<br />

a history <strong>of</strong> chronic narcotic use secondary to his inguinal pain. He<br />

reports that he can press on the mass and make it disappear, but the<br />

maneuver is extremely painful, with pain score increasing to 10/10 in<br />

severity. <strong>The</strong> pain has been present since the operation. It worsens<br />

throughout the day, starting with fairly little pain in the morning and<br />

crescendoing toward the evening. It radiates into his scrotum. Physical<br />

activity makes the pain unbearable. He denies any electrical shock or<br />

burning sensation in the groin or skin.<br />
