THE EFFECT OF THE SUGARBAKER PROCEDURE ON HAEMOSTASIS<br />

Pseudomyxoma peritonei is a progressive disease <strong>of</strong> <strong>the</strong> peritoneum,<br />

characterised by <strong>the</strong> gradual accumulation <strong>of</strong> mucinous fluid in <strong>the</strong> peritoneum.<br />

Left untreated it is ultimately fatal. Although no cure exists,<br />

<strong>the</strong> Sugarbaker procedure combining complete cytoreduction, intraoperative<br />

hyper<strong>the</strong>rmic intra-peritoneal chemo<strong>the</strong>rapy and post-operative<br />

intra-peritoneal chemo<strong>the</strong>rapy, has shown promising results. The<br />

aim <strong>of</strong> this study was to assess haemostatic function in this group <strong>of</strong><br />

patients pre-operatively and <strong>the</strong> effect <strong>of</strong> <strong>the</strong> Sugarbaker procedure on<br />

haemostasis. Citrated plasma was collected from 40 patients, pre and<br />

post operatively, and pre and 30 minutes post any blood components<br />

during surgery. Coagulation screening tests and whole blood global assay<br />

for haemostasis were performed. The prothrombin time (PT), activated<br />

partial thromboplastin time (APTT), fibrinogen (Fib) were performed on<br />

<strong>the</strong> MDA II (Trinity Bioscience). Thromboelastometry tests (Ex-TEM, In-<br />

TEM and Fib-TEM) were performed on <strong>the</strong> ROTEM (Biodis). Pre-operatively<br />

patients had normal PT (mean = 13.5s) and APTT (mean = 30.6s)<br />

and raised Fib (mean = 4.7g/L), suggesting <strong>the</strong> possibility <strong>of</strong> a hypercoagulable<br />

state. The PT (mean pre-op=13.5s, post-op= 15.9s), APTT (mean<br />

pre-op= 30.7s post-op= 36.6s), and fibrinogen (mean pre-op= 4.7g/L postop=<br />

1.6g/L) all displayed significant differences with p values <strong>of</strong>
