CLINICAL RELEVANCE OF REGULATORY NETWORK IN THE BONE MARROW<br />

COMPARTMENT OF PATIENTS WITH MULTIPLE MYELOMA<br />

Regulatory network in <strong>the</strong> bone marrow (BM) compartment <strong>of</strong><br />

patients with multiple myeloma (MM) consists <strong>of</strong> functional interplay<br />

between <strong>the</strong> myeloma cells and microenvironment resulting with <strong>the</strong><br />

interaction <strong>of</strong> various cytokines, <strong>the</strong>ir receptors and adhesion molecules.<br />

The aim <strong>of</strong> study was to analyze prognostic significance <strong>of</strong> different<br />

tumor-host interactions in <strong>the</strong> BM <strong>of</strong> patients (pts) with MM by<br />

immunohistochemical markers <strong>of</strong> sensitivity to <strong>the</strong> IL-6; adhesion molecules;<br />

osteoclastogenesis; and angiogenesis. Patients and Methods. Sixty<br />

newly diagnosed MM pts (33 male/27 female pts, mean age 60 years,<br />

range 35-75) were distributed according to <strong>the</strong> clinical stage (CS,<br />

Salmon&Durie) as: I 8pts, II 22pts, III 30pts. IgG myeloma was diagnosed<br />

in 35pts; IgA in 12pts; light chains in 12pts. Regarding ISS score,<br />

<strong>the</strong> group included: ISS1 18pts, ISS2 13pts, ISS3 29pts. All patients were<br />

treated with conventional chemo<strong>the</strong>rapy. All samples <strong>of</strong> BM biopsies<br />

were analyzed for <strong>the</strong> immunohistochemical expression <strong>of</strong> gp 130,<br />

VCAM, OPG and RANKL. The intensity <strong>of</strong> <strong>the</strong>se staining was graded<br />

as weak (0-30% cells), moderate (31-60% cells), and strong (>60% cells).<br />

Analyzing <strong>the</strong> microvessel density (MVD), BM vessels were visualized<br />

by immunohistochemical staining for CD34. The number <strong>of</strong> vessels per<br />

400x high power field (HPF) was counted in <strong>the</strong> area <strong>of</strong> <strong>the</strong> most dense<br />

vascularization. Results. The expression <strong>of</strong> gp130 was higher in III vs. I<br />
