3. CBT Procedures<br />

Notification of CBT courses<br />

Under the conditions of appointment, AMIs must notify DVA Business Support Unit<br />

(BSU) on a monthly basis, when they will be conducting CBT courses. The AMI may<br />

notify us by email at dva.ami@doeni.gov.uk, telephone or in writing, <strong>to</strong> DVA, BSU<br />

Section, Balmoral Road, Belfast, BT12 6QL, telephone number 028 90 541858.<br />

Reporting accidents/incidents<br />

AMIs are required <strong>to</strong> notify BSU section, in writing of <strong>an</strong>y serious or fatal accidents<br />

that may occur during CBT courses. The DVA CBT Incident Report Form, <strong>an</strong> electronic<br />

version of which is available on the Agency’s website should be used <strong>to</strong> report these<br />

incidents/accidents. This form c<strong>an</strong> be completed <strong>an</strong>d returned via e-mail or by post.<br />

In addition <strong>to</strong> notifying us, it is the AMIs responsibility <strong>to</strong> comply with the Reporting<br />

of Injuries, Diseases <strong>an</strong>d D<strong>an</strong>gerous Occurrences Regulations - (NI) 1997 (commonly<br />

referred <strong>to</strong> as RIDDOR). Information c<strong>an</strong> be obtained from:<br />

AMIs ceasing CBT courses<br />

AMIs that cease <strong>to</strong> provide CBT courses should inform DVA, BSU Section in writing<br />

<strong>an</strong>d return the following documentation:<br />

• The original letter of authorisation; <strong>an</strong>d<br />

• All CBT certificates both used <strong>an</strong>d unused. On receipt of all documents, we will<br />

arr<strong>an</strong>ge for a refund <strong>to</strong> be issued on <strong>an</strong>y unused/unspoilt certificates, less <strong>an</strong><br />

administration charge.<br />

Because the above are accountable documents, failure <strong>to</strong> return them <strong>to</strong> DVA BSU,<br />

may result in <strong>an</strong>y future applications being affected or refused.<br />

CBT moni<strong>to</strong>ring visit<br />

DVA has responsibility <strong>to</strong> moni<strong>to</strong>r <strong>an</strong>d regulate the st<strong>an</strong>dard of instruction given by<br />

AMIs in <strong>delivering</strong> CBT courses, ensuring that the courses given comply with the<br />
