4. Appendices<br />

xi. you shall update the pupil’s logbook on completion of each element of CBT<br />

<strong>an</strong>d retain a record for each pupil for a period of not less th<strong>an</strong> two years <strong>an</strong>d<br />

make these records available on request <strong>to</strong> DVA;<br />

xii. records containing personal data must be retained <strong>an</strong>d disposed of under<br />

the principles of the Data Protection Act 1998;<br />

xiii. you shall immediately issue each successful pupil with a CBT certificate on<br />

completion of <strong>an</strong> approved <strong>training</strong> course;<br />

xiv. you shall complete each certificate according <strong>to</strong> instructions supplied by DVA;<br />

xv. you shall retain copies of CBT certificates for a minimum of two years from<br />

the date of issue <strong>an</strong>d shall make available on request a duplicate, <strong>to</strong> a person<br />

whose CBT certificate has been lost, s<strong>to</strong>len or defaced. The maximum fee that<br />

c<strong>an</strong> be charged for a duplicate CBT certificate DL 196NI is £20.00;<br />

xvi. you shall notify DVA Business Support Unit (BSU) on a monthly basis of the:<br />

• number of certificates issued <strong>an</strong>d their serial numbers*;<br />

• number of certificates which are void due <strong>to</strong> mistakes in completion <strong>an</strong>d their<br />

serial numbers (void certificates should be forwarded <strong>to</strong> BSU);<br />

• name(s) of pupils that certificates were issued <strong>to</strong>; <strong>an</strong>d<br />

• driving licence number of each pupil a certificate was issued <strong>to</strong>.<br />

*if no certificate were issued you shall submit a nil return.<br />

xvii. you shall be responsible for the safe <strong>an</strong>d secure s<strong>to</strong>rage of all unused <strong>an</strong>d<br />

completed (copy) CBT certificates <strong>an</strong>d personal data;<br />

xviii.you shall report <strong>an</strong>y loss or theft of CBT certificates <strong>to</strong> the PSNI <strong>an</strong>d DVA<br />

immediately giving full details of the circumst<strong>an</strong>ces <strong>an</strong>d certificate numbers.<br />

If <strong>an</strong>y certificates reported lost or s<strong>to</strong>len are recovered, you must inform the<br />

PSNI <strong>an</strong>d DVA immediately;<br />

xix. you shall deal promptly <strong>an</strong>d efficiently with enquiries <strong>an</strong>d booking of<br />

appointments, keep <strong>an</strong>d submit <strong>to</strong> DVA Business Support Unit (BSU) such<br />

information about the courses conducted as may reasonably be requested;<br />

xx. you shall deal promptly <strong>an</strong>d effectively with <strong>an</strong>y complaint. Should the matter<br />

remain unresolved, the complain<strong>an</strong>t must be advised <strong>to</strong> write <strong>to</strong> the Business<br />
