3. CBT Procedures<br />
If the AMI who issued the original certificate is no longer operating, DVA may issue<br />
a duplicate certificate <strong>to</strong> a trainee, which is why it is import<strong>an</strong>t that you forward<br />
copies <strong>to</strong> DVA BSU.<br />
CBT Certificate Returns<br />
Each book of CBT certificates has a return form enclosed. This form should be<br />
completed, <strong>an</strong>d posted or emailed <strong>to</strong> BSU at DVA (Testing) Headquarters on a<br />
monthly basis. The information should be legible <strong>to</strong> ensure it is not returned <strong>to</strong> you.<br />
If you get <strong>an</strong>y forms sent back <strong>to</strong> you for amendment <strong>an</strong>d it appears the licence<br />
details are incorrect, please either send a second copy of the return form or forward<br />
the pupils name, address <strong>an</strong>d date of birth <strong>to</strong> BSU <strong>an</strong>d we will endeavour <strong>to</strong> trace<br />
the correct Driving Licence number.<br />
