Introduction L’étape ultime de l’IGRT « Image Guided Radiation Therapy » est de recaler au quotidien les faisceaux d’irradiation sur la tumeur juste avant de traiter (mouvements inter fractions) et que ces faisceaux suivent les éventuels mouvements de la tumeur (tracking) pendant le traitement (mouvements intra fractions). Sans marqueur radio-opaque implanté dans la tumeur, cet objectif est aujourd’hui difficile à atteindre, mais des solutions intermédiaires dégradées mais accessibles et efficaces peuvent être envisagées. Dans tous les cas, l’imagerie indispensable pour parvenir à un recalage efficace de la tumeur par rapport aux faisceaux est une imagerie 3D offrant un bon contraste entre les tissus mous et possiblement instalable dans la salle d’irradiation. La solution la plus naturelle a donc consisté à faire de l’imageur portal le détecteur pour une acquisition 3D conique dite Cone Beam Computed Tomography (CBCT) de l’image de transmission de la source de rayons X utilisée pour le traitement à travers le patient. Malheureusement, aux énergies de traitement, le contraste entre les tissus mous est insuffisant et les niveaux de dose engagés pour l’obtention de telles images sont importants (100 à 500 mGy). L’idée réalisable à un coût raisonnable a donc consisté à adjoindre au statif de l’appareil de traitement un tube à rayons X et un capteur plan en opposition par rapport au patient. L’ensemble étant en rotation autour du patient, il devient alors possible d’obtenir par rétroprojection filtrée une imagerie 3D avec un contraste proche de celui obtenu sur un scannographie à rayons X (RX) conventionnel tout en restant sur des niveaux de dose acceptables (10 à 50 mGy). Plus encore qu’en scannographie RX, du fait notamment des contraintes mécaniques induites par le surplus de poids ajouté au statif et de la relative faiblesse du contraste du fait du rayonnement diffusé incohérent, cette imagerie doit faire l’objet de contrôles de qualité réguliers de manière à conserver le bénéfice de cette imagerie vis-à-vis de la précision géométrique du traitement par rapport au coût engendré par le patient du fait des doses de rayons X que cette imagerie engage. Dans la suite de l’introduction nous allons décrire le système d’acquisition CBCT. Dans la deuxième partie nous allons présenter des outils logiciels pour un contrôle automatique de la qualité des images CBCT en termes de contraste, résolution spatiale, bruit et uniformité, taille de pixel et distorsions géométriques, épaisseur de coupe et linéarité du signal. Les résultats obtenus sont présentés dans la section 3. 3.1.1. Acquisition « Cone Beam » Plusieurs solutions d’imagerie tomographique (CT) ont été introduites pour l’utilisation clinique ; nous citons par exemple, (a) le scanner mobile (Cheng et al. 2003, Ma and Paskalev 2006) qui consiste à installer un scanner dans la salle de traitement, (b) la tomographie « cone beam » basse énergie (kV CBCT) fixe (Jaffray et al. 2002, Oelfke et al. 2006, Moore et al. 2006) qui utilise l’imageur embarqué kV, ou mobile (Sorensen et al. 2006), (c) la tomographie « cone beam » haute 81
