• Dur<strong>in</strong>g <strong>the</strong> descent <strong>in</strong>to Karratha, <strong>the</strong> crew <strong>of</strong> <strong>the</strong> Sikorsky S76 took avoid<strong>in</strong>g<br />

action when <strong>the</strong>y observed a Eurocopter Super Puma climb<strong>in</strong>g towards <strong>the</strong>m. The<br />

helicopters were 200 m laterally and 100 ft vertically apart. The crew <strong>of</strong> <strong>the</strong> Super<br />

Puma was operat<strong>in</strong>g on <strong>the</strong> <strong>in</strong>correct frequency.<br />

7.2 Consequential events<br />

In <strong>the</strong> 70 occurrences where consequential events were known, <strong>the</strong>re were 59 missed<br />

approaches and go-arounds, and 11 rejected take<strong>of</strong>fs. These types <strong>of</strong> precautionary<br />

avoid<strong>in</strong>g action were generally taken when ano<strong>the</strong>r aircraft was sighted on <strong>the</strong> runway<br />

strip dur<strong>in</strong>g f<strong>in</strong>al approach, when ano<strong>the</strong>r aircraft was observed dur<strong>in</strong>g f<strong>in</strong>al approach<br />

to be on f<strong>in</strong>al approach to <strong>the</strong> reciprocal runway, or when two aircraft came <strong>in</strong>to close<br />

proximity on base leg or f<strong>in</strong>al. In addition to <strong>the</strong>se occurrences (where <strong>the</strong><br />

consequential event was due to a precautionary act), <strong>the</strong>re were eight 31 o<strong>the</strong>r<br />

occurrences <strong>in</strong> which a collision occurred (four collisions on ground, four mid-air<br />

collisions).<br />

More than half <strong>of</strong> <strong>the</strong> occurrences where a consequential event was recorded (n = 58)<br />

<strong>in</strong>volved at least one passenger transport aircraft; however, only one <strong>of</strong> <strong>the</strong> eight<br />

collisions <strong>in</strong>volved passenger transport aircraft (both conduct<strong>in</strong>g charter operations).<br />

In total, occurrences <strong>in</strong>volv<strong>in</strong>g passenger transport aircraft led to 48 missed<br />

approaches and go-arounds, n<strong>in</strong>e rejected take<strong>of</strong>fs, and one collision.<br />

31 Seven <strong>of</strong> <strong>the</strong>se occurrences were <strong>the</strong> seven accidents and serious <strong>in</strong>cidents <strong>in</strong>volv<strong>in</strong>g damage and/or<br />

<strong>in</strong>jury discussed <strong>in</strong> section7.1. The additional occurrence was an <strong>in</strong>cident which <strong>in</strong>volved a collision<br />

on ground between two taxi<strong>in</strong>g de Havilland DH.82 Tiger Moth aircraft at Heck Field (ALA), Qld.<br />

This collision did not <strong>in</strong>volve any <strong>in</strong>juries and resulted <strong>in</strong> only m<strong>in</strong>or damage to both aicraft, and<br />

hence is coded as an <strong>in</strong>cident.<br />
