Section 4 – Documentation of Field Activities: Documents the performance of the field<br />

activities related to the Site Investigation, <strong>and</strong> summarizes this information for each<br />

respective REC.<br />

Section 5 – Endangerment Assessment: Provides an assessment of field observations <strong>and</strong><br />

laboratory results, including the significant physical features of the Site <strong>and</strong> the general<br />

distribution, nature, degree <strong>and</strong> extent of <strong>site</strong> contamination for each respective REC. The<br />

assessment is also used (as necessary) to more specifically define (or revise) Site RECs.<br />

Section 6 – Remediation Objectives Evaluation: Discusses RECs, COCs, potential receptors<br />

<strong>and</strong> exposure pathways. This section presents any potential exposure route exclusions <strong>and</strong><br />

provides a brief “pre-<strong>remediation</strong>” TACO evaluation, including a discussion of applicable<br />

exposure routes <strong>and</strong> related evaluations, <strong>and</strong> then discusses the Tier 1 <strong>and</strong> Tier 2<br />

Remediation Objectives (ROs) selected for closure of the Site are also discussed. The<br />

section briefly discusses those areas requiring <strong>remediation</strong>, as well as the institutional<br />

controls required to achieve specified <strong>site</strong> ROs.<br />

Section 7 – Conclusions: Summarizes the findings of the CSI/ROR, <strong>and</strong> provides a<br />

discussion of proposed remedial actions at the Site based on these findings, the RA’s goals<br />

<strong>and</strong> the planned future use of the Site. It is intended this section provide a general roadmap<br />

of the RA’s plans for resolving the identified Site RECs, as they relate to the intended reuse<br />

of the Site.<br />

Section 8 – Licensed Professional Engineer (LPE) Affirmation: Affirmation by the LPE<br />

directing the <strong>investigation</strong> <strong>and</strong> <strong>remediation</strong> of the Site.<br />

Together the aforementioned sections of the <strong>report</strong> are intended to satisfy the requirements of<br />

35 IAC Section 740.420, 740.425, 740.440 <strong>and</strong> 740.445 to provide a complete presentation of<br />

historical data <strong>and</strong> <strong>investigation</strong>s related to the Site.<br />
