h<strong>and</strong>ling the sample. COC forms are included in the appendices of <strong>report</strong>s, <strong>and</strong><br />

therefore shall have as professional an appearance as possible.<br />

Samples will be packaged <strong>and</strong> transported in a manner that maintains the integrity of the<br />

sample <strong>and</strong> permits the analysis to be performed within the prescribed holding time.<br />

Prior to shipment, each sample container will be inspected for a label with the proper<br />

sample identification code.<br />

Samples will be either h<strong>and</strong>-delivered or shipped via overnight mail to the laboratory, or<br />

picked-up by a laboratory representative. The COC forms will be sealed in a plastic bag<br />

<strong>and</strong> transported inside the sample cooler. In addition, any shipping receipts will be<br />

incorporated into the COC documentation. Samples will be packed in the cooler using<br />

bubble-wrap packing materials. Any samples suspected of being highly contaminated<br />

will additionally be sealed in Ziploc ® -type bag. The cooler will be taped closed using<br />

custody seals provided by the laboratory to prevent tampering during transport. Upon<br />

relinquishing the sample cooler, V3 field personnel will sign custody of the samples over<br />

with the time <strong>and</strong> date at the bottom of the COC form. V3 will retain one copy of the<br />

COC documentation <strong>and</strong> the laboratory will retain a second copy.<br />

COC labels <strong>and</strong> forms provide immediate <strong>and</strong> necessary information for the processing<br />

of samples by the laboratory, <strong>and</strong> establish historic documentation of the sampling<br />

endeavor <strong>and</strong> tracking of samples, enabling defense of analytical data in a court of law.<br />

The COC form serves as a link to the information provided on the sample labels, <strong>and</strong><br />

gives the lab overall information on the sample shipment (number of samples sent,<br />

number of sampling containers, date, time, <strong>and</strong> other pertinent information). If<br />

discrepancies are noted, these can be resolved early, when memories are still clear.<br />

Visual observations or specific data on sampling locations or intervals may also be<br />

helpful in future review of data.
