ElectronicProtected Health Information shall mean PHI thatis transmitted by or maintained in<br />

electronic media as that term is defined in 45 CFR 162 103<br />

Limited Data Set shall have the same<br />

meaning as the term limited data set as set forth in as<br />

defined in 45 CFR 164 514 e 2<br />

Privacy Rule shall mean the Standards for Privacy ofIndividually Identifiable Health Information<br />

45 CFR Parts 160 and 164 Subparts A and E<br />

Protected Health Information or PHI shall have the same m aning as the term protected<br />

health information as set forth in the Privacy Rule limited to the infonnation created orreceived by<br />

Connecticut General from or on behalf of the Plan provided<br />

that Protected Health Information<br />

shall not include i education records covered by the Family Educational Rights and Privacy Act as<br />

amended 20 USC 1232g ii records described in 20USC 1232g a 4 B iv and Hi employment<br />

records held by the Plan in its role as employer<br />

Secretary<br />

Services<br />

shall mean the Secretary of the United States Department<br />

of Health and Human<br />

Security Incident shall have the same meaning as the term<br />

security incident as set forth in 45<br />

CFR 164 304<br />

Unsecured Protected Health Information<br />

shall mean PHI that is not secured through the use<br />

of a<br />

technology or methodology specified by the Secretary in accordance with the requirements<br />

of Section 13401 h l ofthe HITECH Act<br />
