right to re evaluate the Savings Target
Guarantee should Employer s disease
prevalence be lower than CIGNA Book
ofBusiness average by more than five 5
percent and should the medical and
pharmaceutical cost paid PDMPM be
below CIGNA Book ofBusiness by more
than ten 10 percent
4 Evaluation ofPerformance and Pavment

provide to Employer a written report
using the Well Aware Program Report
Methodology see Table 1 with respect
to the Disease Management Guarantees
within one hundn d eighty 180 days
following the end ofthe Guarantee
