This letter confirms that Connecticut General Life Insurance Company Connecticut General<br />
agrees that beginning July 1 <strong>2010</strong> and terminating June 30 2011 the Grace Period for the<br />
payment of monthly billed charges is extended from thirty one 31 to six y 60 days<br />
1 If Employer fails to pay the monthly billed charges Connecticut GI neral may<br />
immediately terminate this deferral arrangement<br />
2 Connecticut General may terminate this arrangement upon thirty 30 days prior notice<br />
