applicants need to be a resident <strong>of</strong> Bernalillo<br />
county, bring pro<strong>of</strong> <strong>of</strong> income, rent, bank<br />
statement, etc. Also a photo ID is required.<br />

Free basic eye care and glasses for children,<br />
adults and seniors in low-income, working families<br />
who do not have vision coverage.<br />

Medical and dental services provided at same<br />
building, however the medical and dental<br />
administration and services are separate. Medical<br />
services include routine medical screening,<br />
education, and emergency referrals.<br />
Dental care includes primary dental care and is<br />
based on set fees/sliding scale.<br />
Accepts most insurance, Medicare and SALUD.<br />
