3 Yorkshire and Humberside Partnership PriorityGroup on <strong>Transition</strong>The Yorkshire and Humberside Partnership ‘From Child to Adult’ PriorityGroup provides an active regional focus <strong>for</strong> improving how young disabledpeople and their families experience the transition from children’s to adultservices. Every LA has at least one representative on the group. There arealso reps from Health, the voluntary sector, Parent Partnership, CAMHS,LSC, Valuing People, FE, Job Centre Plus, Centre <strong>for</strong> Learning Excellence,Connexions and Government Office.The aims of the Group are to develop and share good practice; support thedelivery of key areas of policy; and provide intelligence and expertise onissues affecting disabled children and young people at transition, to directlysupport and in<strong>for</strong>m policy making. The Group is facilitated by two externalconsultants. It meets quarterly, as do three sub groups on: person-centredtransition reviews, families and support <strong>for</strong> employers. The Group is fundedby the Regional Partnership.Appendix 4: Practice examples:Yorkshire and Humberside PartnershipRecent and current work (2007/8)Person-centred transition reviews group• Sharing good practice on reviews including the work done in Calderdalewhere education reviewing officers are trained in person-centredtransition reviews• Encouraging local authorities to take advantage of the free trainingoffered by the Valuing People Person-centred <strong>Transition</strong> ReviewProgramme• Involvement in a regional conference on transition organised by theCouncil <strong>for</strong> Disabled Children, DfES and the Office <strong>for</strong> Disability Issueswith Valuing People.Support <strong>for</strong> employersWe are working on:• Mapping good practice in employment of young disabled peoplethroughout the region• Finding out what works well• In<strong>for</strong>mation to employers about employing young disabled people, basedon the social model of disability• Expressing the importance of college courses preparing young peoplewith the skills local employers want.FamiliesAnother sub group is looking at what support families need at transitionincluding relevant, accessible and timely in<strong>for</strong>mation. An in<strong>for</strong>mation projectinvolving family members is currently being planned.Appendix 4What’s happening around the regionWe are mapping transition work throughout the region as a way of sharingknowledge and contacts.
