Some local parent partnershipservices and children’s in<strong>for</strong>mation services can provide in<strong>for</strong>mation on thelocal transition process and some options after school. However, to try andbetter meet the needs of disabled young people, many local authoritieshave produced a ‘transition pack’ <strong>for</strong> families which gives in<strong>for</strong>mation onthe local transition protocol, potential specialist support services and widergeneral support services.<strong>Transition</strong> pack content<strong>Transition</strong> packs should aim to:• be accessible• give a clear and easy-to-follow route through the transition process• provide a glossary of terms and words used• set out the roles and responsibilities of all agencies involved to ensure thateveryone is clear• provide in<strong>for</strong>mation about where young people and parents can receivefurther help and advice. 48Generally, local transition packs will cover the following areas as standard:• what happens during the Year 9 transition review process• what is available <strong>for</strong> parents and families• which local agencies are involved in the <strong>for</strong>mal transition process• what responsibilities these agencies have, <strong>for</strong> example; Connexionschildren’s and adult social care services, health agencies and education• available options after school including Further Education provision, WorkBased Learning and day centre options• what benefits and other financial support is available• any services and support offered by local voluntary, private andcommunity agencies• details on local housing options• leisure and play services.48 CDC transition mapresearch, unpublishedIn<strong>for</strong>mation <strong>for</strong> young peopleHaving a guide to local support and services available to disabled young58
