Leicester City: Policy and Planning Officer – <strong>Transition</strong>The LSC has funded this post, which is a resource <strong>for</strong> all partneragencies to encourage collaboration and inter-agency workingpractices to support transition activities. The post is hosted inChildren and Young People’s <strong>Services</strong> and reports to the<strong>Transition</strong>s Working Group, a subgroup of the Learning DisabilityPartnership Board (LDPB), and the CYPSPB.The transitions action plan is closely aligned with other strategicand working plans, including the CYPP, Adult Learning DisabilitiesPlan, the LSC’s priorities <strong>for</strong> disabled learners and the ConnexionsService Business Plan. Some of the activities underway include:• development of a transitions pathway plan• person-centred review programme and introducing personcentredthinking skills to schools• development of a CD-Rom ‘Going to college’, based on whatyoung people say is important to them to meet theirin<strong>for</strong>mation needs• best practice activities – in<strong>for</strong>mation transfer, linked to theperson-centred review activities• proposals to develop a DVD <strong>for</strong> parents/carers focusing on theirengagement in the transition process and the changes fromchildren’s to adult services.Further examples of effective practice are available fromwww.transitioninfonetwork.org.uk.Quality standards checklistApproaching work with disabled young people in transition from aparticipative, multi-agency perspective can go a long way towardmaximising outcomes. Checklist B1 in Appendix 6 gives a brief overview ofissues to consider when looking at developing an effective multi-agencystrategy <strong>for</strong> providing support at transition.Chapter 2: Overview of statutory servicesChildren’s trusts and multi-agency workingChecklist B2 in Appendix 6 lists the key issues to consider in ensuring theprocess of transition is on each agencies agenda and has a place in the localCYPP and the actions attached to it. Addressing change at this level providesfoundations <strong>for</strong> developing operational strategy to support individual youngpeople and their families when they need and on their terms.Key legislation, guidance and resourcesThe children’s trust arrangements have their foundations in the ChildrenAct 2004. Although the Act does not set out a specific duty <strong>for</strong> localauthorities to develop a children’s trust, there is an expectation thatauthorities will use the duty to cooperate set out in the Act to develop achildren’s trust.
