The Multimedia Profiling worker fromPaddington Arts works closely with local authority staff to ensurethat their work is meeting the needs of disabled young people. Formore in<strong>for</strong>mation go to www.paddingtonarts.org.uk.As a result of the transition review meeting in Year 9, actions should be agreedbetween the young person, their family, the school and other relevant agencies.On the basis of this, the head teacher has responsibility <strong>for</strong> drawing up a planthat clearly defines who is responsible <strong>for</strong> what and when actions should betaken. Currently, the Connexions service has a responsibility <strong>for</strong> overseeing thedelivery of this transition plan. This plan must be reviewed and updated eachyear, in consultation with the disabled young person and their family.Out-of-area placementsSome disabled young people attend special schools or are looked afteroutside of their local authority area. For this group, the transition planningprocess will involve the school they attend and any local health services theyare receiving, as well as their placing local authority. The process will alsoinvolve the Connexions service local to their school and the one in their localarea. The head teacher will be the person responsible <strong>for</strong> notifying theplacing authority of the upcoming transition review. This can presentparticular issues <strong>for</strong> staff wanting to support disabled young people.Travelling, sometimes considerable distances, to visit and then attend thetransition review, proves to be a real challenge <strong>for</strong> staff from all agencies.Where young people are looked after it is important that the transitionplanning process is co-ordinated with the statutory process <strong>for</strong> reviewingthe looked after care plan. This is essential in ensuring that disabled youngpeople get the support they want and need. With this in mind, a number oflocal authorities have developed ‘home and host’ arrangements. Theexample below from Westminster illustrates how this can work whereConnexions acts as the lead agency.Chapter 1: The transition process<strong>Transition</strong> reviewsYoung people in placements outside of WestminsterIt is expected that the Connexions PA linked to the out-ofborough(‘host’) placement will provide the ongoing support <strong>for</strong>the young person and their family. However, the WestminsterConnexions PA in the young person’s home area should workclosely with their ‘host’ colleague and also attend annual reviewswhere necessary (e.g. Year 9 and Year 11) to ensure the transitionprocess is running smoothly.The Connexions PA will need to ensure that the transition plans<strong>for</strong> those disabled young people in schools out of borough arebrought to the attention of the Young People’s <strong>Transition</strong> (YPT)operational group <strong>for</strong> future planning of supportive servicesfollowing the young person’s 18th birthday.13
