5 The Quest – <strong>for</strong> improved services in <strong>Transition</strong>to Adulthood in LeicesterIntroductionThis article considers the problem of developing interagency transitionpathways <strong>for</strong> disabled and special needs young people. It reviews the natureof the problem and the requirements of parent/carers and young people. Thewriter looks at the process that has led to an agreed interagency pathway inLeicester. Finally the essay examines some of the future work that is needed.The mountain – the problem<strong>Transition</strong> to adulthood has always been a large mountain, covered in cloudand high winds. (Some would say a good deal of hot air as well!). Themountain, when viewed from different sides leads to various different,appreciations of the geography. This can lead to different routes to thesummit. Typically we discovered that, Education, Further Education Colleges,Health and Social Care all had a different appreciation of the topography.The key issue to resolve such differences was that Parent/Carers and Youngdisabled adults all made it absolutely clear that we had to climb themountain – roped together. The aim, there<strong>for</strong>e, was to agree a pathway upthe mountain, to reduce complaints and unnecessary disputes – we had totrust and rely on each other.Base campHaving agreed to climb the pinnacle some key elements of the leadershipwere assembled. The Connexions Senior Manager <strong>for</strong> Learning Disabilitiesand Disabilities, the LD Head of Service in Social <strong>Services</strong> and the ServiceManager <strong>for</strong> Disabled Children agreed to lead the developments.A series of early (rather than easy) wins were identified. These were smaller,but challenging peaks. The idea was that by working together we woulddevelop mutual trust and confidence as well as a team approach: all wouldrecognise the same initiatives and achievements. Examples of these foothillsare:• Regular transition in<strong>for</strong>mation events – 14 to date• <strong>Transition</strong>s In<strong>for</strong>mation Packs that are issued each year• Commitment to a programme of in<strong>for</strong>mation sharingAppendix 4: Practice examplesImproving services in LeicesterThe foothillsWe have been surveying this mountain <strong>for</strong> four years; it is a major climbrather than a ramble.
