Like transition itself, developing and reviewing a protocol is an ongoingprocess which needs to be monitored and evaluated regularly. The transitionplanning group needs to include a representative number of local disabledyoung people and their families, representatives from children’s services,adult social services, health services, local colleges and the Connexionsservice as a minimum. It may also have representatives from other agenciesincluding the Children’s In<strong>for</strong>mation Service, independent schools andleisure services. In most areas, this group meets a few times a year to discusshow local provision is working and how it can be improved. Some localareas have access to regional transition networks, supported by regionalpartnerships, which offer a <strong>for</strong>um to share practice and learning with awider group. See Appendix 4 <strong>for</strong> an example.Effective practiceThe process of bringing a group of appropriate people together to plan,agree and implement a local strategic transition protocol is in itself achallenging piece of work. Each agency has a range of different goals andpriorities at any given time so bringing the right people together can taketime and commitment. It is there<strong>for</strong>e crucial to have a person and agencyleading in this process.SandwellChapter 1: The transition process<strong>Transition</strong> protocolsA <strong>Transition</strong> Team Manager, employed by Prospects <strong>Services</strong> Ltd,funded through Black Country Connexions has established a multiagency <strong>Transition</strong> Team to support disabled young people throughtheir transition from education to post 16 options. The managerset up and chaired a multi agency <strong>Transition</strong> Steering Group, tookon the role of <strong>Transition</strong> Champion <strong>for</strong> Sandwell LearningDisability Partnership Board and promoted the regionallydeveloped <strong>Transition</strong> Pathway with all agencies.The <strong>Transition</strong> Manager chaired a sub group with representativesfrom the SEN team, mainstream and special school staff andmainstream and LDD Connexions Personal advisers to developjoint guidelines to be used in transition planning within Sandwell.The SEN team purchased copies of the <strong>Transition</strong> Pathwaymaterials <strong>for</strong> all schools and multi agency training sessions on theuse of the <strong>Transition</strong> Pathway materials and on the jointguidelines was delivered in July. Use of the materials and planningprocesses have been recommended to all schools.Prospects have put together guidelines on the role of theConnexions Personal Advisers which incorporates the use of thepathway’s person-centred ‘Getting Ready Sheets’ to use withyoung people in their work.5
