As well as giving them a sense ofownership, this ensures that in<strong>for</strong>mation included is relevant and of interestto those intending to use it.Leicester City spent time working with the local parent carercouncil and professionals from a range of agencies to producetransition guidance <strong>for</strong> local disabled young people and theirfamilies. The work was led by the Connexions service and involvedall agencies offering support at transition. This included localvoluntary sector organisations.The guidance pack includes in<strong>for</strong>mation on a wide range of localservices, details of what local agencies can offer, and a clear‘pathway’ chart to outline the transition process.
