Evidencefrom the pathfinder stage of children’s trusts development clearly showsthat in areas where time was taken to develop agreed strategies acrossagencies the ensuing work and services were improved. 6 Most areas willalready have some strategic agreements in place about who provides whichservices <strong>for</strong> disabled young people and their families at transition. This is the main groupresponsible <strong>for</strong> developing and producing a multi-agency transition protocol.Brighton and Hove have a transition <strong>for</strong>um to serve this purpose. The<strong>for</strong>um reports into children’s services and to the LDPB so that adult servicesand planning are also in<strong>for</strong>med.
