New Graduate Student and new Undergraduate Research Student Safety Orientation Announcement, in August of each year: DEPARTMENTAL SAFETY TRAINING SESSION Tuesday, August 21 1:00 – 2:30 pm Salem 210 Mike Thompson Bruce King This session will be held as a Chemical Safety Orientation for new Graduate Students in the Chemistry, Physics, and NanoTechnology Departments who work with chemicals. Chemistry Department Purchase Order Request System Chemicals, equipment, and supplies are usually ordered from outside private companies with the use of two types of Purchase Order Forms as follows: A) The Department‟s new and improved computer generated Purchase Request Form, prepared by Ms. Melissa Doub, the Business Manager for the Chemistry Department, is located at: http://www.wfu.edu/chemistry/purchase/. Please bookmark this page and remember that you must log on to it the same way you log on to the Chemical Inventory system (that is, with your username, minus the @wfu.edu, and your password). If you need to see an example of the way to fill out the form, speak to Melissa Doub in office # 208, top floor. Purchases for over $1000 cannot be placed this way. B) TYPE IN ALL PERTINENT INFORMATION ON THE DEPARTMENTAL ORDER FORM � Starting at the top of the form, type in the Date you wish to place the order over the phone. � Fill in the “Ship To” field with your Research professor‟s name, or with a combination of your professor‟s name and your own. For example: 1. Dr. Albus Dumbledore, or 2. Dr. Albus Dumbledore/Harry Potter, or, for that matter, Dr. Albus Dumbledore/Potter is fine. But please! Always include your research Professor’s name in the shipping address. Do not address Chemistry Department related purchases to your name alone!!! Graduate students are sometimes difficult to track down and find in the building! � Fill in the “Bill to Account” field. This is the professor's grant number or funding description, from which said purchase will be paid. The faculty member authorizing the purchase will know this number or abbreviation/acronym. His/her permission is required for obtaining this information. If this is a departmental purchase for a teaching lab, you must fill out this field, with, say, Dept, and scroll to “yes” in the “Departmental Charge?” field. � Fill out the “Purchase Order To” field with the name of the company – its Vendor Address is optional. � Fill in the “Company’s WFU Account No.” Most firms accepting large volume business from our department will have their own particular "account number" for all University‟s that buy from them, in order to expedite their invoicing (billing) records. Fisher Scientific Company's account number for us is 861448-01, for example. For Sigma-Aldrich Chemical Company it is 49464319 (unless you have an individual PIN number). SEE THE VENDER’S LIST OF OTHER COMPANY ACCOUNT NUMBERS AT THE END OF THIS SECTION. If you know this number, please include it, since the company will usually ask for it when you are calling in an order. If you don't, the lab manager will add it, if you wish him to place the order. Sometimes the company already knows what this number is if you‟re calling from Wake University Chemistry Department. Discounts offered to us will be recognized by the company with use of this number. � Fill in the company's telephone number (toll-free, if available) � You won‟t need to add the Salesperson Name in most cases. � Use the Notes field to add any important information to the form regarding the order that you may wish to refer to later, such as items on back-order, special shipping instructions, original catalog numbers no longer available for items that have received newer catalog numbers, substitute descriptions, extra discount pricing information, technical information regarding possible instrumental problems, questions regarding purity of chemicals, etc. � You can only order up to 20 different catalog items per order. Start out with item # 1. Supply the Cat. No. (catalog Number) for the item. Write in the QTY (for quantity of items – That is, the number of bottles, or the number of packs, or the number of cases of the item). Lastly, type in the Description of the item. If it is a chemical, type in the amount, unit, and chemical name. For example, type in 100 grams of 4-Nitroaniline, or one case of 6 X 500 grams of Sodium hydroxide, etc. If you are ordering anything other than chemicals, type in its description and how much of it you want. For example, type in “3 cases of test tubes”, or “5 packs of nitrile gloves”, etc. 173
