Attachment 1HEALTH AND COMMUNITY SUPPORTS59. Continue to pilot and expand innovative EMS projects designed to respond to the needs <strong>of</strong>older adults in the community (e.g., CREMS project, Vial <strong>of</strong> Life project).60. Identify and utilize existing service delivery mechanisms to enhance services to older adultsand/or to identify older adults at risk, e.g.:• Train postal service staff, pharmacists, neighbours, EMS, Snow Angels, etc. torecognize when older adults may be at risk• A “gatekeeper” pilot project using formal and informal neighbourhood groupsto provide guidance for neighbours in supporting and checking on their olderneighbours (see recommendation #33)• Expand the existing Snow Angels program (see recommendation #8)• Broaden Snow Angels mandate to provide home maintenance help and support• Support a peer support model <strong>of</strong> seniors assisting seniors61. Work in partnership with community agencies to support the distribution <strong>of</strong> a resource guidefor caregivers to help them navigate and know what services are available in <strong>Guelph</strong>.62. Review <strong>City</strong> policy and revise eligibility as necessary to recognize that their employees whoare caregivers can live outside the home <strong>of</strong> the person needing the care.63. Identify and define the <strong>City</strong>’s role in the provision <strong>of</strong> health services to <strong>City</strong> residents.64. Work in partnership with GWSA and other health providers who <strong>of</strong>fer such services asawareness, education, immunization (e.g., flu clinics), foot care – the <strong>City</strong> role to contribute topromotion and to provide convenient locations that older adults can easily access – e.g.community centres, libraries, <strong>City</strong> Hall65.
