6.1 SITE RISK ANALYSIS LAUSD Proposed South Region High School #9 & Middle School #4 6.0 REMEDIATION PHASE 6-1 January 2007 Site Specific Health & Safety Plan (HASP) Before work begins, Project Managers lead a team that performs a risk analysis at each work site to identify hazards that require specific control measures. During weekly progress meetings, the Project Engineer and subcontractors submit written summaries of upcoming work tasks and associated risks and control measures to the Project Manager using Exhibit 6-1. The weekly summaries identify upcoming mobilization or demobilizations tasks, audits and inspections, competent person changes, training and new activities requiring an Activity Hazard Analysis (AHA). Subcontractors add activities to these summaries at least two weeks in advance of the work. Potential hazards are listed below. • Chemical exposures (Table 6-1) • Environmental – cold/heat stress, animals, insects, poisonous plants/vegetation • Hazardous material handling • Heavy equipment operation • Noise • Traffic The safety and health risk analysis presented in this section is for anticipated field activities. All tasks are expected to be performed using Level D protection levels. See Section 10 for details of PPE. Any future activities not included in this HASP will be addressed in task-specific workplans. Overall Hazard Rating for this Work: Low to Medium The following describes the potential major tasks and the potential health and safety hazards for each task. Task 1 – Asphalt Cap Maintenance Work: Hot mixed asphaltic concrete will be unloaded from a truck and applied with hand tools and a hand-operated vibratory compactor. Potential Hazards - Heavy equipment hazards from being struck by/against moving equipment; being caught and injured from moving equipment parts; lifting hazards from lifting and carrying materials; slip, fall, and trip hazards from walking on slippery and uneven surfaces; cuts, skin abrasions, and bruises from handling of sharp tools/objects; vibration hazards; potential exposures to hot asphalt material; and heat stress when working in a hot environment. Task 2 – Groundwater monitoring: Consists of geophysical logging of wells, well maintenance, well abandonement, well drilling and installation, well development, groundwater sampling, and aquifer pump testing. • Geophysical logging – Consists of lowering equipment into the well to record subsurface soil characteristics. 192
