violence against a car which was in <strong>the</strong> room that he only lived a few<br />

minutes after being- injured. He leaves a wife <strong>and</strong> four children. This<br />

accident happened June 29.<br />

Thomas Greeley, a driver, age 19 years <strong>and</strong> single, was instantly killed<br />

in Ivil mine, July 11, by being caught between <strong>the</strong> cars. The deceased<br />

had ga<strong>the</strong>red part <strong>of</strong> his trip <strong>and</strong> in addition to Avhich he wanted to hitch<br />

on a water wagon, <strong>and</strong> to do so, he hooked <strong>the</strong> tail chain to <strong>the</strong> water<br />

wagon to bring it up close enough to <strong>the</strong> o<strong>the</strong>r part <strong>of</strong> <strong>the</strong> trip to hitch<br />

on, <strong>and</strong> it is supposed that <strong>the</strong> velocity with which <strong>the</strong>y were coming toge<strong>the</strong>r<br />

caused <strong>the</strong> hitching to jolt out <strong>and</strong> his head was caught between<br />

<strong>the</strong> sideboards <strong>of</strong> <strong>the</strong> water car <strong>and</strong> <strong>the</strong> full car, as <strong>the</strong> body was found<br />

at right angles with <strong>the</strong> road <strong>and</strong> immediately under <strong>the</strong> side <strong>of</strong> <strong>the</strong> car.<br />

Gareviuo Carchilli was instantly killed by a fall <strong>of</strong> slate in entry 2,<br />

room 99, Walton's upper road, on July 14. At <strong>the</strong> time <strong>of</strong> <strong>the</strong> accident,<br />

Carchilli was beariug-in when a piece <strong>of</strong> slate 5yV feet long 1-^-(j wide<br />

<strong>and</strong> 13 inches thick fell upon him. In examining <strong>the</strong> room I found it<br />

troubled with a horse-back which made <strong>the</strong> slate somewhat dangerous.<br />

It seems that <strong>the</strong>y had a slate post under <strong>the</strong> slate, but being in <strong>the</strong> road<br />

for bearing-in it was removed; <strong>the</strong> removing <strong>of</strong> this post no doubt<br />

caused <strong>the</strong> accident. The deceased was considered a practical miner, but<br />

it seems that he had overlooked this part <strong>of</strong> <strong>the</strong> room <strong>and</strong> did not think<br />

it dangerous. His age was 30 years. He leaves a wife <strong>and</strong> one child.<br />

Edward Ward was fatally injured in Cincinnati mines, July 18, by being<br />

caught between a car <strong>and</strong> bottom. The deceased was riding out <strong>of</strong><br />

<strong>the</strong> mine between <strong>the</strong> hitchings on a loaded trip, <strong>and</strong> it is supposed that<br />

<strong>the</strong> hitching came out letting him fall, when <strong>the</strong> rear car caught him between<br />

it <strong>and</strong> <strong>the</strong> rail. He died July 22 : age 55 years.<br />

On July 22, Thomas Warren, a boy about 13 years <strong>of</strong> age, while working<br />

with his fa<strong>the</strong>r in Walton's Upper mine was instantly killed by a fall<br />

<strong>of</strong> slate in entry "C," room 8. The slate that fell on deceased was 5| feet<br />

long 3^(7 feet wide <strong>and</strong> 9 inches thick. At <strong>the</strong> time <strong>of</strong> <strong>the</strong> accident <strong>the</strong><br />

boy was knocking coal, <strong>and</strong> it is supposed that in doing this, <strong>the</strong> slate<br />

was loosened sufficiently to cause it to swing <strong>and</strong> discharge a post which<br />

was set under it. An inquest was held <strong>and</strong> a verdict <strong>of</strong> accidental death<br />

rendered.<br />

On August 18, Domonic Domendio, a miner, employed in <strong>the</strong> Bunola<br />

mines, while taking down slate was struck by a piece <strong>of</strong> it on <strong>the</strong> right<br />

leg, injuring <strong>the</strong> member in such a manner that an amputation was per-<br />

formed, but <strong>the</strong> shock to <strong>the</strong> system at <strong>the</strong> time <strong>of</strong> <strong>the</strong> accident wcs too<br />

great, as he died some four weeks after being injured. He leaves a wife<br />

<strong>and</strong> three children.<br />

On September 5th Mr. E. F. Miller, superintendent <strong>and</strong> mine-boss <strong>of</strong><br />

<strong>the</strong> Nottingham mine, was fatally injured by <strong>the</strong> falling <strong>of</strong> <strong>the</strong> approach<br />

to <strong>the</strong> tipple which carried him down with <strong>the</strong> wreck. He lived about<br />

four hours after being hurt. In my investigation <strong>of</strong> this accident, I
