2 .3 .1 Overview of<br />

recent drug trends<br />

The drug of choice for drug <strong>use</strong>rs <strong>in</strong> Ch<strong>in</strong>a<br />

is hero<strong>in</strong>, followed by benzodiazep<strong>in</strong>es<br />

(<strong>in</strong>clud<strong>in</strong>g triazolam, diazepam, clonazepam<br />

and nitrazepam), o<strong>the</strong>r opiates such as<br />

morph<strong>in</strong>e and opium, cannabis, ketam<strong>in</strong>e<br />

and amphetam<strong>in</strong>e-type substances (ATS),<br />

such as ‘ice’ and ‘ecstasy’ . In Ch<strong>in</strong>a’s grow<strong>in</strong>g<br />

urban centres, <strong>use</strong> of MDMA (‘ecstasy’) has<br />

become <strong>in</strong>creas<strong>in</strong>gly popular while cannabis<br />

<strong>use</strong> is widespread <strong>in</strong> X<strong>in</strong>jiang . The <strong>use</strong> of ATS<br />

was rare until 1995 but is currently expand<strong>in</strong>g<br />

rapidly: <strong>the</strong>re is a perception that ecstasy<br />

and methamphetam<strong>in</strong>e are <strong>the</strong> most popular<br />

drugs consumed .<br />

2 .3 .2 Prevalence of drug <strong>use</strong><br />

The Government of Ch<strong>in</strong>a acknowledges<br />

that <strong>the</strong> number of drug <strong>use</strong>rs is much<br />

higher than those who are registered . As of<br />

late 2003, <strong>the</strong> number of registered drug<br />

<strong>use</strong>rs was 1,050,000, a 15-fold <strong>in</strong>crease,<br />

account<strong>in</strong>g for 0 .081 per cent of Ch<strong>in</strong>a’s<br />

total population . Officially, some suggest<br />

<strong>the</strong> number of drug <strong>use</strong>rs ranges from an<br />

estimated 6–8 million; unofficially, estimates<br />

are as high as 12 million . However, it must<br />

be stated this is not based on scientific or<br />

epidemiological studies . The estimated<br />

number of <strong>in</strong>ject<strong>in</strong>g drug <strong>use</strong>rs (IDUs) ranges<br />

from a low of 356,000 to 3,500,000 with a<br />

mid-range figure of 1,928,000 .<br />

2 .3 .3 <strong>Drug</strong> supply<br />

The grow<strong>in</strong>g of illicit opium is m<strong>in</strong>or, and<br />

manufactur<strong>in</strong>g of hero<strong>in</strong> with<strong>in</strong> Ch<strong>in</strong>a’s<br />

national borders has not yet been identified<br />

. Most hero<strong>in</strong> produced <strong>in</strong> Myanmar is<br />

reportedly trafficked through Ch<strong>in</strong>a, which<br />

has become an important transhipment route<br />

for <strong>the</strong> <strong>in</strong>ternational market . This proliferation<br />

of drug traffick<strong>in</strong>g <strong>in</strong> Ch<strong>in</strong>a largely emerged as<br />

a result of new routes from Afghanistan <strong>in</strong>to<br />

western Ch<strong>in</strong>a, particularly X<strong>in</strong>jiang Prov<strong>in</strong>ce .<br />

Cannabis is grown ma<strong>in</strong>ly <strong>in</strong> X<strong>in</strong>jiang and<br />

Yunnan, primarily for domestic <strong>use</strong> .<br />

The ephedr<strong>in</strong>e plant grows wild <strong>in</strong> many<br />

parts of north-east Ch<strong>in</strong>a and <strong>the</strong> chemical<br />

extracted from <strong>the</strong> plant is processed for<br />

pharmaceutical purposes . Methamphetam<strong>in</strong>e<br />

laboratories are located <strong>in</strong> various prov<strong>in</strong>ces<br />

along <strong>the</strong> eastern and south-eastern coastal<br />

areas . Ch<strong>in</strong>a <strong>in</strong> recent years has become a<br />

major source of methamphetam<strong>in</strong>e for many<br />

Sou<strong>the</strong>ast <strong>Asia</strong>n and <strong>Pacific</strong> rim nations .<br />

Ch<strong>in</strong>ese law authorities also note a surge<br />

<strong>in</strong> <strong>the</strong> production of MDMA, mostly for<br />

<strong>the</strong> domestic market <strong>in</strong> Beij<strong>in</strong>g, Shanghai,<br />

Nanj<strong>in</strong>g, Guangzhou and Shenzhen . Ch<strong>in</strong>a<br />

also produces <strong>the</strong> chemical potassium permanganate,<br />

known to be sold to Lat<strong>in</strong> America to<br />

ref<strong>in</strong>e coca<strong>in</strong>e .<br />

2 .3 .4 <strong>Drug</strong>-tak<strong>in</strong>g practices,<br />

risk factors and trends<br />

Smok<strong>in</strong>g of hero<strong>in</strong> is still popular but <strong>the</strong><br />

trend towards <strong>in</strong>ject<strong>in</strong>g is well established .<br />

Ecstasy is generally taken orally or by smok<strong>in</strong>g,<br />

while ‘ice’ is usually <strong>in</strong>haled or smoked:<br />

<strong>the</strong>re are reports of <strong>in</strong>ject<strong>in</strong>g methamphetam<strong>in</strong>e,<br />

sometimes mix<strong>in</strong>g it with hero<strong>in</strong>, but<br />

it is difficult to gauge how widespread <strong>the</strong><br />

practice is . Reports show that unsafe <strong>in</strong>ject<strong>in</strong>g<br />

is widespread among drug <strong>use</strong>rs, <strong>the</strong> greatest<br />

risk factor be<strong>in</strong>g <strong>the</strong> shar<strong>in</strong>g of syr<strong>in</strong>ges .<br />

2 .3 .5 Summary table<br />

Sexual risk behaviours among drug <strong>use</strong>rs are<br />

alarm<strong>in</strong>g, with multiple sexual partners common<br />

and <strong>the</strong> rate of condom <strong>use</strong> <strong>in</strong> sex be<strong>in</strong>g<br />

low . By 2002, HIV was found among drug<br />

<strong>use</strong>rs <strong>in</strong> all 31 prov<strong>in</strong>ces, autonomous regions<br />

and municipalities . In 2001, 70 per cent of<br />

all HIV cases were found among IDUs, but<br />

as of 2003 <strong>the</strong> proportion of reported HIV<br />

among IDUs had dropped to 44 per cent: IDU<br />

rema<strong>in</strong>s <strong>the</strong> ma<strong>in</strong> HIV transmission route <strong>in</strong><br />

Ch<strong>in</strong>a . The prevalence of hepatitis C among<br />

IDUs was mostly above 70 per cent .<br />
