Situational analysis of illicit drug issues and responses <strong>in</strong> <strong>the</strong> <strong>Asia</strong>–<strong>Pacific</strong> region<br />

Nationwide <strong>the</strong>re are few activities aimed at<br />

drug <strong>use</strong>rs to assist <strong>the</strong>m to re<strong>in</strong>tegrate <strong>in</strong>to<br />

<strong>the</strong> community follow<strong>in</strong>g treatment . Evaluations<br />

of current treatment practices are scant<br />

and systematic follow-up data are poor . It is<br />

generally agreed that <strong>the</strong> relapse rates are very<br />

high, with estimates <strong>in</strong> <strong>the</strong> range of 80–100<br />

per cent (Economic and Social Commission<br />

for <strong>Asia</strong> and <strong>the</strong> <strong>Pacific</strong>, 2003b; Human Rights<br />

Watch, 2003; Yap et al ., 2002) .<br />

A methadone ma<strong>in</strong>tenance treatment (MMT)<br />

pilot program began <strong>in</strong> 2003, with <strong>the</strong><br />

support of <strong>the</strong> M<strong>in</strong>istry of Health, <strong>the</strong> M<strong>in</strong>istry<br />

of Public Security, and <strong>the</strong> State Food and<br />

<strong>Drug</strong> Adm<strong>in</strong>istration . In 2004, <strong>the</strong> first phase<br />

commenced <strong>in</strong> eight cl<strong>in</strong>ics <strong>in</strong> five prov<strong>in</strong>ces:<br />

Yunnan, Guizhou, Sichuan, Zhejiang and<br />

Guangxi . All MMT patients pay a daily fee<br />

of RMB5–10 (US$1 .20) . The pilot project<br />

has 2000 drug <strong>use</strong>rs enrolled . Results have<br />

been promis<strong>in</strong>g, with a reduction <strong>in</strong> <strong>in</strong>ject<strong>in</strong>g<br />

behaviour <strong>in</strong> <strong>the</strong> last month from 70 per cent<br />

to 1 per cent and reduction of needle shar<strong>in</strong>g<br />

<strong>in</strong> <strong>the</strong> last three months from 14 per cent to<br />

3 .5 per cent . The MMT Work<strong>in</strong>g Group is now<br />

review<strong>in</strong>g 31 applications for an additional<br />

26 national cl<strong>in</strong>ics <strong>in</strong> an expansion of <strong>the</strong><br />

pilot sites . Based on <strong>the</strong> large number of drug<br />

<strong>use</strong>rs <strong>in</strong> Ch<strong>in</strong>a, it has been estimated that<br />

1000–1500 methadone cl<strong>in</strong>ics are required,<br />

and <strong>the</strong>se are planned to be <strong>in</strong> operation<br />

by 2007 (Harm Reduction Work<strong>in</strong>g Group,<br />

2004) . At <strong>the</strong> end of 2005 it is anticipated<br />

<strong>the</strong>re will be 100 MMT programs operat<strong>in</strong>g <strong>in</strong><br />

various parts of Ch<strong>in</strong>a (Wu, 2005) .<br />

O<strong>the</strong>r harm reduction strategies appear to be<br />

ga<strong>the</strong>r<strong>in</strong>g pace and recently <strong>the</strong> government<br />

announced that it hoped to expand outreach<br />

to 60 per cent of drug <strong>use</strong>rs with<strong>in</strong> five years<br />

through 210 additional outreach centres .<br />

There has been an <strong>in</strong>creased attempt to<br />

educate drug <strong>use</strong>rs about HIV transmission at<br />

detoxification centres by <strong>the</strong> Department of<br />

Public Security at some pilot sites or by <strong>in</strong>ter-<br />

national NGOs . Harm reduction approaches<br />

appear to be ga<strong>in</strong><strong>in</strong>g gradual acceptance;<br />

some local authorities have embarked upon<br />

needle and syr<strong>in</strong>ge programs (NSPs): Yunnan<br />

legalised this approach <strong>in</strong> a March 2004 law .<br />

Similar programs have been implemented<br />

<strong>in</strong> six o<strong>the</strong>r prov<strong>in</strong>ces <strong>in</strong> areas where HIV is<br />

most prevalent among IDUs . Ch<strong>in</strong>ese NSPs<br />

function <strong>in</strong> three ways: by encourag<strong>in</strong>g drug<br />

<strong>use</strong>rs to purchase new <strong>in</strong>ject<strong>in</strong>g equipment<br />

from pharmacies, to obta<strong>in</strong> free needles or<br />

vouchers at exchange centres (Thompson,<br />

2004) . In 2004, Y<strong>in</strong><strong>in</strong>g City, X<strong>in</strong>jiang,<br />

severely affected by HIV among IDUs,<br />

launched NSPs <strong>in</strong> three communities us<strong>in</strong>g<br />

17 street outlets (<strong>in</strong>clud<strong>in</strong>g health cl<strong>in</strong>ics,<br />

small shops and specialty outlets) and mobile<br />

services . A total of 22,943 needle and syr<strong>in</strong>ge<br />

packages have been distributed and up to<br />

24,000 <strong>use</strong>d needles have been collected and<br />

disposed of <strong>in</strong> cremation facilities . As a result<br />

of this program, <strong>the</strong> shar<strong>in</strong>g of needles has<br />

decreased by 52 per cent and condom <strong>use</strong><br />

has <strong>in</strong>creased by 44 per cent among IDU<br />

couples (Ni et al ., 2006) .<br />

O<strong>the</strong>r responses<br />

School-based education<br />

It is reported that schools and universities<br />

throughout <strong>the</strong> country promote counternarcotics<br />

education by provid<strong>in</strong>g sem<strong>in</strong>ars<br />

and workshops on <strong>the</strong> dangers of drugs; <strong>the</strong>se<br />

also <strong>in</strong>clude an HIV/AIDS component . There<br />

have been various education programs such<br />

as ‘100 Model Schools for <strong>Drug</strong> Preventive<br />

Education’ among middle school students,<br />

youth community law schools and ‘Stay<strong>in</strong>g<br />

away from <strong>Drug</strong>s’ by community youngsters<br />

(National Narcotics Control Commission and<br />

Public Security, 2004c; Silk Road Studies<br />

Program, 2004) . The authors have not been<br />

able to access details of <strong>the</strong>se programs .<br />
