NURSING PRIORITY NO. 5. To promote wellness (Teaching/<br />

Discharge Considerations):<br />

• Discuss factors related to occurrence of deficit, as individually<br />

appropriate. Early identification of risk factors can decrease<br />

occurrence and severity of complications associated with<br />

hypovolemia.<br />

• Identify and instruct in ways to meet specific nutritional<br />

needs.<br />

• Instruct client/SO(s) in how to measure and record I/O, monitor<br />

fluid status.<br />

• Identify actions (if any) client may take to correct deficiencies.<br />

• Review/instruct in medication regimen and administration<br />

and interactions/side effects.<br />

• Instruct in signs and symptoms indicating need for immediate/further<br />

evaluation and follow-up care.<br />

• Individual findings, including factors affecting ability to manage<br />

(regulate) body fluids and degree of deficit.<br />

• I/O, fluid balance, changes in weight, urine specific gravity,<br />

and vital signs.<br />

• Results of diagnostic testing/laboratory studies.<br />

• Plan of care and who is involved in the planning.<br />

• Teaching plan.<br />

IMPLEMENTATION/EVALUATION<br />

• Client’s responses to treatment/teaching and actions performed.<br />

• Attainment/progress toward desired outcome(s).<br />

• Modifications to plan of care.<br />

DISCHARGE PLANNING<br />

• Long-term needs, noting who is responsible for actions to be<br />

taken.<br />

• Specific referrals made.<br />
