Criteria—Individual Will:<br />

• Develop plan to address individual risk factors for progression<br />

of illness/sequelae.<br />

• Demonstrate effective problem solving in integration changes<br />

of therapeutic regimen into lifestyle.<br />

• Identify/use available resources.<br />

• Remain free of preventable complications/progression of<br />

illness and sequelae.<br />

Actions/Interventions<br />

NURSING PRIORITY NO. 1.To assess situation and individual<br />

needs:<br />

• Ascertain client’s knowledge/understanding of condition and<br />

treatment needs. Note specific health goals.<br />

• Identify individual’s perceptions of adaptation to treatment/<br />

anticipated changes.<br />

• Note treatments added to present regimen and client’s/SO’s<br />

associated learning needs.<br />

• Discuss present resources used by client to note whether<br />

changes need to be arranged (e.g., increased hours of home<br />

care assistance; access to case manager to support complex/long-term<br />

program).<br />

NURSING PRIORITY NO. 2.To assist client/SO(s) in developing<br />

strategies to meet increased demands of therapeutic regimen:<br />

• Identify steps necessary to reach desired health goal(s).<br />
