• Achieve normal/acceptable elimination pattern emptying<br />

bladder/voiding in appropriate amounts.<br />

• Alter lifestyle/environment to accommodate individual<br />

needs.<br />

Actions/Interventions<br />

NURSING PRIORITY NO. 1. To assess status and adaptive skills being<br />

used by client:<br />

• Identify physical conditions (e.g., surgery, childbirth,<br />

recent/multiple pregnancies, pelvic trauma, neurogenic bladder<br />

from central nervous system disorders or neuropathies<br />

[stroke/spinal cord injury, diabetes], mental/emotional dysfunction,<br />

prostate disease/surgery) that can impact client’s<br />

elimination patterns.<br />

• Determine client’s usual pattern of elimination and compare<br />

with current situation. Review voiding diary, if indicated.<br />

Provides baseline for future comparison.<br />

• Observe voiding patterns, time, color, and amount voided, if<br />

indicated (e.g., postsurgical or postpartum client) to document<br />

normalization of elimination.<br />

• Ascertain methods of self-management (e.g., limiting or<br />

increasing liquid intake, acting on urge in timely manner,<br />

established voiding schedule, regularly spaced catherization)<br />

to identify strengths/areas of concern in elimination management.<br />
