by questions, statement of misconception, failure to improve<br />

on previous regimen, and development of preventable complications.<br />

risk for Activity Intolerance: risk factors may include imbalance<br />

between myocardial O2 supply and demand, and cardiac depressant<br />

effects of certain drugs (-blockers, antidysrhythmics).*<br />

range of therapeutic effectiveness, lack of education/proper precautions,<br />

acute Pain may be related to distention/rupture of fallopian tube, possibly<br />

evidenced by verbal reports, guarding/distraction behaviors, facial mask<br />

of pain, and autonomic responses (diaphoresis, changes in vital signs).<br />

risk for deficient Fluid Volume [isotonic]: risk factors may include hemorrhagic<br />

losses and decreased/restricted intake.*<br />

Anxiety [specify level]/Fear may be related to threat of death and possible<br />

loss of ability to conceive, possibly evidenced by increased tension,<br />

apprehension, sympathetic stimulation, restlessness, and focus on self.<br />

Eczema (dermatitis) CH<br />

Pain/[Discomfort] may be related to cutaneous inflammation and irritation,<br />

possibly evidenced by verbal reports, irritability, and scratching.<br />

risk for Infection: risk factors may include broken skin and tissue<br />

trauma.*<br />

Social Isolation may be related to alterations in physical appearance,<br />

possibly evidenced by expressed feelings of rejection and decreased<br />

interaction with peers.<br />

Edema, pulmonary MS<br />

excess Fluid Volume may be related to decreased cardiac functioning,<br />

excessive fluid/sodium intake, possibly evidenced by dyspnea, presence<br />

of crackles (rales), pulmonary congestion on x-ray, restlessness,<br />

anxiety, and increased central venous pressure (CVP)/pulmonary<br />

pressures.<br />

*A risk diagnosis is not evidenced by signs and symptoms, as the<br />

problem has not occurred and nursing interventions are directed at<br />

prevention.<br />
