MEASURING CHILD<br />

DISABILITY<br />

A society cannot be equitable unless all children are<br />

included, and children with disabilities cannot be<br />

included unless sound data collection and analysis<br />

render <strong>the</strong>m visible.<br />

Measuring child disability presents a unique set<br />

<strong>of</strong> challenges. Because children develop and<br />

learn to perform basic tasks at different speeds,<br />

it can be difficult to assess function and distinguish<br />

significant limitations from variations in<br />

normal development. 114 The varying nature and<br />

severity <strong>of</strong> disabilities, toge<strong>the</strong>r with <strong>the</strong> need to<br />

apply age-specific definitions and measures, fur<strong>the</strong>r<br />

complicate data collection efforts. In addition,<br />

<strong>the</strong> poor quality <strong>of</strong> data on child disability<br />

stems, in some cases, from a limited understanding<br />

<strong>of</strong> what disability is in children and, in<br />

o<strong>the</strong>r cases, from stigma or insufficient investment<br />

in improving measurement. The lack <strong>of</strong><br />

evidence that results from such difficulties hinders<br />

<strong>the</strong> development <strong>of</strong> good policies and <strong>the</strong><br />

delivery <strong>of</strong> vital services. As discussed below,<br />

however, efforts to improve data collection are<br />

under way – and <strong>the</strong> very act <strong>of</strong> ga<strong>the</strong>ring information<br />

is sparking positive change.<br />

Evolving definitions<br />

While <strong>the</strong>re is general agreement that definitions<br />

<strong>of</strong> disability should incorporate both<br />

medical and social determinants, <strong>the</strong> measurement<br />

<strong>of</strong> disability is still predominantly medical,<br />

with a focus on specific physical or<br />

mental impairments.<br />

Estimates <strong>of</strong> disability prevalence vary depending<br />

on what definition <strong>of</strong> disability is used.<br />

Narrow, medical definitions are likely to yield<br />

lower estimates than broader ones that take<br />

into account social barriers to functioning and<br />

participation. 115<br />

One framework for seeing health and disability<br />

within a broader context <strong>of</strong> social barriers is<br />

<strong>the</strong> International Classification <strong>of</strong> Functioning,<br />

Disability and Health (ICF), developed by <strong>the</strong><br />

World Health Organization. 116 This classification<br />

regards disability in two main ways: as a matter<br />

<strong>of</strong> <strong>the</strong> body’s structure and functions, and in<br />

terms <strong>of</strong> <strong>the</strong> person’s activity and participation.<br />

Disability, as defined by <strong>the</strong> ICF, is an ordinary<br />

part <strong>of</strong> human existence. ICF’s definition effectively<br />

mainstreams disability, shifting <strong>the</strong> focus<br />

from cause to effect and acknowledging that<br />

every person can experience some degree <strong>of</strong><br />

disability. The ICF definition also recognizes that<br />

functioning and disability occur in context, and<br />

<strong>the</strong>refore it is meaningful to assess not only bodily<br />

but also societal and environmental factors.<br />

While <strong>the</strong> ICF was principally designed for<br />

adult disability, a classification derived from it,<br />

<strong>the</strong> International Classification <strong>of</strong> Functioning,<br />

Disability and Health for <strong>Children</strong> and Youth<br />

(ICF-CY) takes a step towards incorporating<br />

<strong>the</strong> social dimension by capturing not only <strong>the</strong><br />

impairment but also its effect on children’s functioning<br />

and participation in <strong>the</strong>ir environment.<br />

The classification covers four main areas: body<br />
