(iv) a declaration that <strong>the</strong> Employee has been advised to seek financial advice<br />
regarding <strong>the</strong> arrangement;<br />
(v) calculations used to determine salary deductions;<br />
(vi) details <strong>of</strong> <strong>the</strong> right <strong>of</strong> return to <strong>the</strong> pre-existing employment arrangements;<br />
(vii) o<strong>the</strong>r matters deemed relevant to <strong>the</strong> arrangements; <strong>and</strong><br />
(viii) signatures <strong>of</strong> <strong>the</strong> Employee <strong>and</strong> CEO.<br />
(b) An extended leave agreement is non-renewable. On <strong>the</strong> expiry <strong>of</strong> an existing agreement,<br />
<strong>the</strong> Employee may lodge a new application for approval by <strong>the</strong> CEO.<br />
59.6 Cessation <strong>of</strong> extended leave<br />
(a) An extended leave arrangement may cease in <strong>the</strong> following ways:<br />
(i)<br />
A <strong>the</strong> request <strong>of</strong> <strong>the</strong> Employee on <strong>the</strong> giving <strong>of</strong> 3 months written notice to <strong>the</strong><br />
CEO, provided that approval <strong>of</strong> <strong>the</strong> request is at <strong>the</strong> discretion <strong>of</strong> <strong>the</strong> CEO, based<br />
on operational <strong>and</strong> o<strong>the</strong>r relevant considerations.<br />
(ii) At <strong>the</strong> initiative <strong>of</strong> <strong>the</strong> CEO, on <strong>the</strong> giving <strong>of</strong> 3 months written notice to <strong>the</strong><br />
Employee, along with reasons for <strong>the</strong> cessation.<br />
(iii) The Employee ceases employment with <strong>the</strong> NTPS.<br />
(iv) The Employee moves to a new work area within <strong>the</strong> Agency, or to ano<strong>the</strong>r<br />
Agency (unless <strong>the</strong> new work area or Agency agrees to continue <strong>the</strong><br />
arrangement).<br />
(b) Where an extended leave arrangement ceases in accordance with paragraph (a), <strong>the</strong><br />
Employee will be reimbursed a lump sum payment <strong>of</strong> monies paid within 2 months <strong>of</strong> <strong>the</strong><br />
date <strong>of</strong> cessation, provided that where <strong>the</strong> Employee has already commenced <strong>the</strong> period<br />
<strong>of</strong> extended leave, he or she will be reimbursed monies paid on a pro-rata basis, in<br />
accordance with <strong>the</strong> portion <strong>of</strong> monies relating to <strong>the</strong> unused period <strong>of</strong> leave.<br />
Division 5 – O<strong>the</strong>r<br />
60. Redeployment <strong>and</strong> Redundancy<br />
The Redeployment <strong>and</strong> Redundancy provisions as set out in Schedule 3 to this Agreement apply in<br />
<strong>the</strong> NTPS.<br />
61. Pupil Nurse<br />
Over <strong>the</strong> term <strong>of</strong> this Agreement <strong>the</strong> Department will explore options for creating employment<br />
opportunities for pupil <strong>nurses</strong> in consultation with Employees <strong>and</strong> <strong>the</strong>ir representatives.<br />
