This instrument is easy <strong>to</strong> administer <strong>and</strong> is reported <strong>to</strong>usually take less than five minutes <strong>to</strong> complete. For patients with limited stamina this <strong>for</strong>mmay be an effective instrument <strong>for</strong> moni<strong>to</strong>ring symp<strong>to</strong>ms in an in- or out-patient setting.I.
