we tested the hypothesis that dually diagnosed patients (i.e. suffering both from
schizophrenia and cannabis abuse) perform worse in neuropsychological tests than
schizophrenic patients without cannabis abuse. On the whole, schizophrenic patients
performed worse than healthy control subjects. Surprisingly, rather than
deteriorating neuropsychological performance, regular cannabis abuse prior to the
first psychotic episode improved cognition in some tests. This was even more
pronounced when regular consumption started before the age of 17. On the other
hand, cannabis use deteriorated test performance in healthy controls, especially in
cases when regular consumption started before the age of 17. To sum up, regular
cannabis abuse has a different effect on cognitive function in schizophrenic patients
and healthy controls.
“
Collectively,
these findings suggest that schizophrenia patients with comorbid
cannabis use disorders (CUD) may represent a higher functioning subgroup of
schizophrenia. Future prospective studies are needed to elucidate the nature of this
relationship.
En studie från 2012 visar att det finns ett samband mellan sårbarhet för psykoser i tidig ålder och
cannabisrökning [38]
Forskare driver en hypotes om huruvida en obalans i det endocannibinoida receptorsystemet ligger
bakom schizofreni [54][55][56] .
“
According to this hypothesis stimulation of the endocannabinoid system would cause
psychotic symptoms, while the blockade of this system could treat schizophrenia.
The current study shows, that in single patients stimulation of CB1 receptors may
result in an improvement of psychosis. It is remarkable that the improvements were
observed in patients with severe disease, who did not respond to other medications.
It can be supposed that the high response rate observed is based on the selection of
patients. Authors noted that the non-response to standard medication with
dopamine-blocking substances might indicate that the psychotic symptoms of these
patients were caused by changes in other systems such as the endocannabinoid
system.
Cannabinoiden CBD har visat sig ha antipsykotiska egenskaper [58][59][60]
Och cannabismedicinen dronabinol reducerar också negativa effekter hos personer med schizofreni:
“
We
are reporting improvement of symptoms of schizophrenia in a small group of
patients who received the cannabinoid agonist dronabinol (synthetic delta-9-
tetrahydrocannabinol). Before this report, cannabinoids had usually been associated
with worsening of psychotic symptoms. In a heuristic, compassionate use study, we
found that 4 of 6 treatment-refractory patients with severe chronic schizophrenia
but who had a self-reported history of improving with marijuana abuse improved
with dronabinol. This improvement seems to have been a reduction of core psychotic
symptoms in 3 of the 4 responders and not just nonspecific calming. There were no
clinically significant adverse effects. These results complement the recent finding
that the cannabinoid blocker rimonabant does not improve schizophrenic symptoms
