However, every respondent formulated recommendations at the <strong>in</strong>dividual patient level<br />

or ‘micro’-recommendations <strong>in</strong>stead of more general or ‘macro’-recommendations.<br />

Most of these recommendations are still dom<strong>in</strong>ated by the character<strong>is</strong>tics of the<br />

current payment systems and organ<strong>is</strong>ation.<br />

Acute care unit<br />

• Rehabilitation (physical therapy, occupational therapy) can start <strong>in</strong> th<strong>is</strong><br />

• As soon as possible transfer to a rehabilitation organ<strong>is</strong>ation for a<br />

multi-d<strong>is</strong>cipl<strong>in</strong>ary rehabilitation programme.<br />

• Multid<strong>is</strong>cipl<strong>in</strong>ary rehabilitation programme<br />

• One price per medical team for multid<strong>is</strong>cipl<strong>in</strong>ary <strong>in</strong>terventions<br />

(neurolog<strong>is</strong>t, orthopaed<strong>is</strong>t, physiatr<strong>is</strong>t, urolog<strong>is</strong>t, … ).<br />

When the <strong>in</strong>dividual can perform transfers, <strong>is</strong> <strong>in</strong>dependent for ADL and adaptations at<br />

home are performed, outpatient rehabilitation <strong>in</strong> the rehabilitation organ<strong>is</strong>ation can<br />

start.<br />

When function<strong>in</strong>g at home <strong>is</strong> <strong>with</strong>out problems, rehabilitation at home can start.<br />

After six months of rehabilitation <strong>in</strong> a rehabilitation organ<strong>is</strong>ation special<strong>is</strong>ed <strong>in</strong> stroke or<br />

non-congenital cerebral lesions, an assessment of dependence must be performed.<br />

Depend<strong>in</strong>g on the results of the assessment the patient can stay <strong>in</strong> the special<strong>is</strong>ed<br />

organ<strong>is</strong>ation or can be transferred to a general rehabilitation organ<strong>is</strong>ation.<br />

General rehabilitation organ<strong>is</strong>ation<br />

Duration of the stay <strong>in</strong> a rehabilitation organ<strong>is</strong>ation depends on the level of recovery<br />

and the social situation. If there <strong>is</strong> recovery of function and difficulties concern<strong>in</strong>g social<br />

situation ex<strong>is</strong>t, early transfer to a nurs<strong>in</strong>g home <strong>with</strong> ma<strong>in</strong>tenance physical therapy <strong>is</strong>
