4 REGISTERED DATA AND PATIENT PROFILES<br />

4.1 BACKGROUND<br />

In chapter 3 an extensive description has been made of the different scales used to<br />

measure outcomes of rehabilitation <strong>with</strong><strong>in</strong> the scope of organizational/f<strong>in</strong>anc<strong>in</strong>g purposes.<br />

Rehabilitation scales and/or datasets are already <strong>in</strong> use <strong>in</strong> some countries <strong>in</strong> order to<br />

classify patients for organizational/f<strong>in</strong>anc<strong>in</strong>g purposes.<br />

<strong>The</strong> approval of ICF as an <strong>in</strong>ternationally accepted classification of function<strong>in</strong>g, has<br />

stimulated the analys<strong>is</strong> of ex<strong>is</strong>t<strong>in</strong>g measures and scales. Major efforts on th<strong>is</strong> part are be<strong>in</strong>g<br />

done by the “ICF branch of WHO”. A systematic review by th<strong>is</strong> group 118 made an <strong>in</strong>ventory<br />

of different measures used <strong>in</strong> acute and post-acute rehabilitation and compared it to ICF.<br />

<strong>The</strong> authors conclude that at th<strong>is</strong> stage no real standard validated scales measur<strong>in</strong>g (cl<strong>in</strong>ical<br />

and functional) character<strong>is</strong>tics of rehabilitation patients are readily <strong>available</strong> that cover<br />

most/all aspects of ICF. At the same time ICF-concepts covered by the <strong>in</strong>dividual<br />

rehabilitation scales vary a lot. Th<strong>is</strong> means that currently, no ex<strong>is</strong>t<strong>in</strong>g validated<br />

rehabilitation scale <strong>is</strong> able to differentiate rehabilitation units and pathologies on bas<strong>is</strong> of<br />

the (cl<strong>in</strong>ical and functional) character<strong>is</strong>tics of patient populations, and at the same time fit<br />

to the ICF-model.<br />

<strong>The</strong> use of ICF itself as a managerial and f<strong>in</strong>ancial tool <strong>is</strong> still under development (see<br />

chapter 3); and many problems still have to be solved.<br />

Tak<strong>in</strong>g <strong>in</strong>to account these observations, th<strong>is</strong> chapter has a practical purpose. In Belgium,<br />

the only formal reg<strong>is</strong>tration system currently used <strong>in</strong> the hospital sector, <strong>in</strong>clud<strong>in</strong>g<br />

<strong>in</strong>dicators of functionality of the patient, <strong>is</strong> the m<strong>in</strong>imum nurs<strong>in</strong>g data set. A quick scan of<br />

literature was performed to assess to what degree current nurs<strong>in</strong>g reg<strong>is</strong>tration systems<br />

could be used to describe and compare patient populations <strong>in</strong> rehabilitation sett<strong>in</strong>gs and<br />

to what degree these datasets could be used for rehabilitation managerial purposes. In a<br />

second part we illustrate how profiles of Belgian m<strong>in</strong>imum nurs<strong>in</strong>g data can differentiate<br />

the character<strong>is</strong>tics of rehabilitation facilities. A third part br<strong>in</strong>gs some examples of<br />

<strong>in</strong>ternationally accepted. Rehabilitation scales that are <strong>in</strong> use <strong>in</strong> some Belgian rehabilitation<br />

centres and sometimes are already used for organizational purposes at the level of the<br />

own hospital.<br />

4.2 A QUICK SCAN OF AVAILABLE LITERATURE<br />

4.2.1 Methods<br />

A quick scan of the databases Medl<strong>in</strong>e (through PubMed), CDC, CRD, Econlit and C<strong>in</strong>ahl<br />

was done <strong>in</strong> November 2006.<br />

Follow<strong>in</strong>g search terms were used: Rehabilitation, Rehabilitation Science, Rehabilitation<br />

research, Rehabilitation centers, D<strong>is</strong>ability evaluation, Severity of d<strong>is</strong>ability, International<br />

classification of function<strong>in</strong>g, d<strong>is</strong>ability and health or d<strong>is</strong>ability, Patient classification,<br />

m<strong>in</strong>imum data set, Research <strong>in</strong>struments, Rehabilitation or Cl<strong>in</strong>ical assessment tools,<br />

outcome assessment, Functional assessment , Rehabilitation patients, FIM(keyword),<br />

subacute care, Barthel Index, D<strong>is</strong>ability evaluation, Functional status, Functional<br />

assessment, Instrument validation, Cl<strong>in</strong>ical assessment tools, Bra<strong>in</strong> <strong>in</strong>juries, or Cerebral<br />

Vascular accident or Activities of daily liv<strong>in</strong>g, Physiotherapy, Physical therapy.<br />

<strong>The</strong> search was general. <strong>The</strong> selection of the articles was based on the title and abstract.<br />

As could be expected, the majority of literature found on nurs<strong>in</strong>g data sets focuses on the<br />

use of datasets for nurs<strong>in</strong>g <strong>is</strong>sues and on the ability to use large cl<strong>in</strong>ical nurs<strong>in</strong>g datasets to<br />

assess the effectiveness of nurs<strong>in</strong>g <strong>in</strong>terventions. <strong>The</strong>re <strong>is</strong> an extensive literature on<br />

nurs<strong>in</strong>g term<strong>in</strong>ology, on <strong>in</strong>formation models and standards for nurs<strong>in</strong>g datasets h . All<br />
