long-stay beds and claim that one <strong>in</strong> ten hospital beds <strong>is</strong> still occupied by someone who<br />
<strong>is</strong> medically ready to be d<strong>is</strong>charged and who should be treated <strong>in</strong> primary care or at<br />
home. In 2004, a committee delivered recommendations on improv<strong>in</strong>g the boundary<br />
between health and social care, and proposed putt<strong>in</strong>g a greater responsibility on<br />
municipalities to provide <strong>in</strong>tegrated social and health care while giv<strong>in</strong>g them the ability<br />
to hire their own doctors if they feel that the counties are not allocat<strong>in</strong>g enough<br />
physician time to municipal home care (SOU, 2004).<br />
8.6.2.3 F<strong>in</strong>anc<strong>in</strong>g rehabilitation<br />
<strong>The</strong> f<strong>in</strong>anc<strong>in</strong>g of the post-acute stroke rehabilitation centres <strong>is</strong> organ<strong>is</strong>ed <strong>in</strong> a fixed<br />
budget system. (contrary to the acute sector that <strong>is</strong> f<strong>in</strong>anced on a DRG-system).<br />
Purchaser-provider negotiation decide on the amount of care there will be offered for<br />
the budget. <strong>The</strong> budget <strong>in</strong>cludes <strong>in</strong>frastructure; e.g hospital beds, clean<strong>in</strong>g etc, staff<strong>in</strong>g,<br />
extra exam<strong>in</strong>ations (such as a new MR exam<strong>in</strong>ation), tra<strong>in</strong><strong>in</strong>g equipment for use at the<br />

(<strong>in</strong>clud<strong>in</strong>g a critical analys<strong>is</strong> of ICF, FIM, FRG, AN-SNAP). <strong>The</strong> project aims at<br />
develop<strong>in</strong>g a method to measure activity and participation of the patient on special care.<br />
But a real wide scale implementation of the ICF <strong>is</strong> not real<strong>is</strong>ed yet. For ICF to become<br />
used more widely there <strong>is</strong> a consensus that more developed guidel<strong>in</strong>es are needed as a<br />
complement to the classification.<br />
<strong>The</strong>re Nordic work group <strong>is</strong> also develop<strong>in</strong>g reflections on how to l<strong>in</strong>k between acute<br />
<strong>in</strong>patient care and rehabilitation and will develop a proposal for rehabilitation <strong>in</strong> 2007.<br />
<strong>The</strong>re <strong>is</strong> yet no l<strong>in</strong>k <strong>with</strong> the current NordDRG systemoo..<br />
8.6.3 Quality<br />
<strong>The</strong> National Board of Health and Welfare <strong>is</strong>sued a set of regulations on quality <strong>is</strong>sues<br />
for all health services to develop cont<strong>in</strong>uous quality improvement. <strong>The</strong> regulations<br />
emphasize on monitor<strong>in</strong>g and quality-improvement measures focus<strong>in</strong>g on technical<br />
quality and safety and <strong>is</strong>sues related to the people for whom health services are<br />
<strong>in</strong>tended.<br />
<strong>The</strong> National Quality Reg<strong>is</strong>ters are used as supportive tools for analyses of the medical<br />
quality and outcomes <strong>in</strong> specific parts of the healthcare system. <strong>The</strong> national quality<br />
reg<strong>is</strong>ters ma<strong>in</strong>ly cover highly special<strong>is</strong>ed care provided at hospitals, whilst primary health<br />
care largely lacks jo<strong>in</strong>t follow-up systems. Standard<strong>is</strong>ed patient questionnaires are used<br />
and Safety problems and shortcom<strong>in</strong>gs <strong>in</strong> care are reg<strong>is</strong>tered, e.g. by the Medical<br />
Responsibility Board, Patient Insurance Fund, and <strong>in</strong> the National Board's R<strong>is</strong>k Database.<br />
Data from National Health Data Reg<strong>is</strong>ters can be used to monitor health care<br />
util<strong>is</strong>ation, morbidity and mortality on a population level. Some 40 national health care<br />
quality reg<strong>is</strong>ters, are developed each conta<strong>in</strong><strong>in</strong>g data on health care outcomes and<br />
treatment for a large number of categories of illness. <strong>The</strong>se reg<strong>is</strong>ters serve as a<br />

knowledge base for cont<strong>in</strong>uous improvement.<br />
Although the prerequ<strong>is</strong>ites for monitor<strong>in</strong>g the quality of care <strong>in</strong> Sweden are good,<br />
further development of models and methods for performance assessment are needed.<br />
Sweden has for <strong>in</strong>stance no databases and quality <strong>in</strong>dicators <strong>in</strong> primary care and care for<br />
many chronic d<strong>is</strong>eases and psychiatric d<strong>is</strong>orders. Hence, the quality of services cannot<br />
be def<strong>in</strong>ed for a large proportion of health services delivered.<br />
In 2001, the National Board of Health and Welfare, jo<strong>in</strong>tly <strong>with</strong> the Federation of<br />
Swed<strong>is</strong>h County Councils and the Swed<strong>is</strong>h Association of Local Authorities started a<br />
reflection on a comprehensive and coherent system for review and for exchang<strong>in</strong>g and<br />
ma<strong>in</strong>ta<strong>in</strong><strong>in</strong>g <strong>in</strong>formation <strong>with</strong><strong>in</strong> treatment and care. Part of the exerc<strong>is</strong>e also focussed on<br />
