results of these measures to ICF scores. F<strong>in</strong>ally, analyse the collected data on their utility<br />
<strong>in</strong> a PCS. Groups <strong>with</strong><strong>in</strong> th<strong>is</strong> PCS must be homogeneous related to required rehabilitation<br />
services and <strong>in</strong>directly to required resources. A comparable approach was respected by<br />
the Div<strong>is</strong>ion of Health Care Policy and Research at the University of Colorado at Denver<br />
for the development of a Uniform Patient Assessment <strong>in</strong>strument for Post-Acute Care.<br />
<strong>The</strong>ir <strong>in</strong>strument <strong>is</strong> <strong>in</strong>tended to cover the population <strong>in</strong> different rehabilitation sett<strong>in</strong>gs and<br />
must facilitate placement dec<strong>is</strong>ion-mak<strong>in</strong>g, enhancement of safety and quality of care<br />
transitions through transm<strong>is</strong>sion of core <strong>in</strong>formation to a receiv<strong>in</strong>g provider and prov<strong>is</strong>ion<br />
of basel<strong>in</strong>e <strong>in</strong>formation for longitud<strong>in</strong>al follow up of health and function 100 .<br />
Key po<strong>in</strong>ts<br />
• <strong>The</strong> conceptual def<strong>in</strong>ition of musculoskeletal and neurological rehabilitation<br />
has to be translated <strong>in</strong>to practice to facilitate dec<strong>is</strong>ions on the organ<strong>is</strong>ation<br />
and f<strong>in</strong>anc<strong>in</strong>g of rehabilitation.<br />
• A patient classification system <strong>is</strong> required to structure <strong>in</strong>flow of <strong>in</strong>dividuals<br />
(What?) and determ<strong>in</strong>e the type and <strong>in</strong>tensity of required <strong>in</strong>terventions<br />
(How?), the professionals to be <strong>in</strong>volved (Who?) and the preferred<br />
organ<strong>is</strong>ation (Where?).<br />
• Outcome measures, outcome models and patient classification systems<br />
ex<strong>is</strong>t. But no l<strong>in</strong>ks are made between outcome measures and outcome<br />
models, or between outcome models and patient classification systems.<br />
• ICF can already be used as a conceptual framework for an outcome model<br />
but the application of ICF <strong>in</strong> cl<strong>in</strong>ical practice and for f<strong>in</strong>anc<strong>in</strong>g purposes only<br />
fits long term v<strong>is</strong>ion. A lot of research <strong>is</strong> go<strong>in</strong>g on about th<strong>is</strong> subject.<br />
• ICF’s success depends on its compatibility <strong>with</strong> measures used <strong>in</strong><br />
rehabilitation and on the improvement of its applicability.<br />
• FIM and Barthel Index are tools measur<strong>in</strong>g the level of dependence related<br />
to activities of daily liv<strong>in</strong>g, but neither FIM nor Barthel Index measure<br />
rehabilitation needs. However, <strong>in</strong> some countries they are used <strong>in</strong> Patient<br />
Classification Systems implemented for resource allocation.<br />
• Most of the ex<strong>is</strong>t<strong>in</strong>g PCS are specific for <strong>in</strong>patient or for outpatient care. An<br />
exception <strong>is</strong> the Australian AN-SNAP, derived from the FIM-FRG-system.<br />
• In order to test ex<strong>is</strong>t<strong>in</strong>g PCS <strong>in</strong> Belgium, datasets from Belgian rehabilitation<br />
patients are necessary.<br />
• A short term option could be to implement the reg<strong>is</strong>tration of FIM or<br />
Barthel Index <strong>in</strong> Belgian rehabilitation centres, <strong>in</strong> order to get some –<br />
although limited- <strong>in</strong>formation on the severity of functional impairment of<br />
rehabilitation patients.<br />
• To l<strong>in</strong>e up <strong>with</strong> <strong>in</strong>ternational tendencies, it could be considered to start a<br />
validation project us<strong>in</strong>g ICF core sets versus FIM or Barthel Index.
