Currently, rehabilitation centres face the problem of treat<strong>in</strong>g patients from different<br />
health <strong>in</strong>surance companies. Sometimes different proofs of quality have to be<br />
demonstrated, lead<strong>in</strong>g to a large overhead and adm<strong>in</strong><strong>is</strong>trative cost 179 Efforts are now on<br />
their way to guarantee a convergence of the different quality assurance programs 180<br />
Reflections have started to take form on the quality of services and quality assurance<br />
programs <strong>in</strong> <strong>in</strong>tegrated care.<br />
Recent efforts are made to ref<strong>in</strong>e the standards used <strong>in</strong> the quality assurance programs.<br />
Some projects are focus<strong>in</strong>g on the study of the structural standards for <strong>in</strong>patient rehab<br />
units treat<strong>in</strong>g patients for musculoskeletal, cardiac, neurological gastroenterological,<br />
oncological, pneumological and dermatological d<strong>is</strong>eases. <strong>The</strong> aim of these projects <strong>is</strong> to<br />
d<strong>is</strong>t<strong>in</strong>gu<strong>is</strong>h between basic criteria that every <strong>in</strong>patient rehab sett<strong>in</strong>g has to fulfill, and the<br />
more specific structural character<strong>is</strong>tics of each of the rehabilitation special<strong>is</strong>ms. Relevant<br />
structural criteria were def<strong>in</strong>ed <strong>in</strong> expert meet<strong>in</strong>gs by means of a modified Delphitechnique<br />
<strong>with</strong> five <strong>in</strong>quiries. 199 "basal criteria" and "assignment criteria" were def<strong>in</strong>ed.<br />
<strong>The</strong> criteria are grouped <strong>in</strong> two doma<strong>in</strong>s: general structural character<strong>is</strong>tics (general<br />
character<strong>is</strong>tics and equipment of rooms; medical/technical equipment; therapy,<br />
education, care; staff) and process-related structures (conceptual frames; <strong>in</strong>ternal quality<br />
management; <strong>in</strong>ternal communication and personnel development). <strong>The</strong> structural<br />
standards are applicable to units for musculoskeletal, cardiac, neurological, oncological,<br />
gastroenterological, dermatological and pneumological rehabilitation 181 <strong>The</strong>se projects<br />
are <strong>in</strong> th<strong>is</strong> stage ma<strong>in</strong>ly research and/or pilot projects.<br />
Integrated care models are be<strong>in</strong>g developed for rehabilitation, taken <strong>in</strong>to account<br />
phases of rehabilitation. A conceptual model has been developed for neurological<br />
rehabilitation<br />
• In Germany, medical post-acute rehabilitation <strong>is</strong> ma<strong>in</strong>ly offered <strong>in</strong><br />
special<strong>is</strong>ed rehabilitation cl<strong>in</strong>ics, although outpatient and part time<br />
<strong>in</strong>patient care has grown considerably.<br />
• Approvals for adm<strong>is</strong>sion to rehabilitation facilities by the <strong>in</strong>surance<br />
companies are needed.<br />
• Quality assurance programmes have been rout<strong>in</strong>ely implemented <strong>in</strong><br />
rehabilitation, most of the time imposed by private <strong>in</strong>surance companies<br />
or sickness funds. <strong>The</strong> statutory health <strong>in</strong>surance has imposed quality<br />
assurance models. Quality assurance programs are <strong>in</strong>tended to impact on<br />
the allocation of patients as well as the f<strong>in</strong>anc<strong>in</strong>g of the rehabilitation<br />
services.<br />
• Patients older than 70 years are less eligible to these models and are<br />
referred to geriatric rehabilitation. Geriatric rehabilitation <strong>is</strong> a special<br />
category of rehabilitation <strong>in</strong> Germany offer<strong>in</strong>g less <strong>in</strong>tense rehabilitation<br />
compared to the other models.
